The Evaluation of Prophylatic Antibiotics about Abdominal Total Hysterectomy

Background: Correctly prophylactic antibiotics using can reduce the antibiotic usage and control medical cost for ATH surgery, and prevent operation site infection or death. Object: By changing antibiotics regimen of ATH surgery, we can try to find the impact on medical care quality and cost-effective benefit. Method: The data collected from the charts of our hospital by the method of case review, to find out the differences in medical care quality and cost-effective between groups. Results: When changing of single dose antibiotics Cefazoline 2gm from Cefazoline 2gm+Gentamycin 80mg, we found no longer use antibiotics after operation. We also found the rates of the wound infection and readmission (returning to ward or to the emergency unit) weren't difference from before. Besides, the regimen could shorten hospital days from 6.20 to 5.77, the cost of drug from NT$1818 to NT$ 1059, and the cost of antibiotics from NT$1044 to NT$402. Been compared the medical cost of ATH to NHI payment, the results showed each case saved NT$330. Conclusions: Correctly prophylactic antibiotics regimens using can reduce the antibiotic usage, it offers the same medical quality care, and controls the medical costs.
