Currently, there are several mental health hospitals in the south part of Taiwan taking over the field of psychiatric home care. In this article, the psychiatric home care nurse assisted one bipolar patient to adjust to life in the community. Through utilizing the skills of observation, intervention, assessment and direct nursing care, the psychiatric home care nurse assessed the main problems that the patient encountered in the community, and connected the patient with needed social services so that the unnecessary cost of social and medical expense can be decreased. It is also hoped that the psychiatric patient's quality of life in the community can be enhanced, and the care-burden of the family can be decreased.
