護理人員交班錯誤率改善方案
Project to Improve the Error Rates during the Nursing Shifts
摘要
正確且完整的交班是護理人員每日必須達成的重要護理活動，本單位常見因交班錯誤導致病患、家屬、醫師、同仁間的抱怨與對護理人員不信任，故本專案旨在降低護理人員交班的錯誤率，經調查發現：現行交班方式所造成的交班平均錯誤率達40.00%，導致交班錯誤的原因為：交班單張設計不符臨床需要、交班單張擺放位置不瑝、單張書寫缺乏一致性，經專案小組進行下列改善：(1)重新修訂交班單張(2)重新規劃交班單張擺放位置(3)制定單張書寫標準(4)擬定單位在職教育(5)建立稽核制度；結果交班平均錯誤率降為11.61%，交班平均滿意度由47.69%提升為70.93%。本專案有效降低了交班錯誤率，其中採用改善措施可提供護理同業作為改善護理人員交班作業及臨床作業規劃之參考。

The accurate and complete nursing shifts are the important nursing activity which nurses are supposed to attain every day. This project aims to reduce the error rates of the nursing shifts. Upon investigation, we find that the error rate nowadays is up to 40%. The possible reasons for the errors are the following: the inadequate design of nursing shift sheets, the lack of the checking system, the lack of unity of the single sheets, the unfounded standard of the nursing shifts, the inadequate specialty for nurses, the unsuitable places for the nursing shift sheets. After the reform, including (1) the revise of suitable nursing shifts sheet, (2) the establishment and checking system of nursing shifts monitoring, (3) the establishment of standard of nursing shift sheets, (4) the revise of the standard shifts, (5) the layout of the on-the-job training, and (6) the renewal program of the items on the nursing shift sheets and the places for the sheets, we found that the error rate descends to 11.61%, the satisfaction raises from 47.69 to 70.93%, and the average waste of time during nursing shifts shortens from 54 minutes to 47 minutes. The enforcement of the project reaches the aim of reducing the error rate of nursing shifts, and it raises the nurses' satisfaction toward nursing shifts as well. It not only raises the nurses' professional literacy but also saves the huge expense of payload.
