探討老人照護人力資本指標之研究:以中部教學醫院護理人力為例

A Study of Exploring the Human Capital Indicators of Elderly Care for Nurse Practitioners: the Case of Central Area Teaching Hospitals in Taiwan

摘要


隨著高齡化社會的來臨,人口老化與疾病慢性化的影響,致使老人照護需求的增加,醫院護理人員是滿足高齡者多元照護需求的第一線實務供給者,有鑑於目前長期照護服務制度與體系,尚未能真確估算現有護理人員於提供老人照護工作時之人力資本,致使醫療機構存在著照護人力資源資源投入與服務績效的不確定性與資訊不對稱。本研究目的為:(1)探討老人照護護理人力資本之現況;(2)探討老人照護護理人力資本指標之類別與項目;(3)提陳應用老人照護護理人力指標之實務意涵。本研究採用描述研究方法,透過文獻探討、實地訪查與問卷調查等程序進行資料之收集,以臺灣中部教學醫院編制內從事老人照護之護理人員作為研究對象,研究工具為「職場學習者對於職場學習能力與氣氛」問卷,得到結論為:未來老人照護之護理人員人力資本指標可由「非財務型」、「人力資源發展型」與「職場學習型」等三層次之人力資本指標進行進一步實證分析,並可結合需求面之老人照護市場相關變數進行均衡分析。

The increase in numbers of the senior citizens and the proportion of people with serious chronic illness in the population has a profound impact in Taiwan. From the service side of elderly and long-term cares are generally more complicated than the ordinary acute care services provided in general practice and hence present a great challenge to health and long-term care system and nurse practitioners. The demand itself in elderly care is of great concern in view of the characteristics of uncertainty and asymmetric information inherited in the medical care facilities. The purpose of this study highlights: (a) demonstrating the human capital of elderly care for nurse practitioners. (b) exploring the human capital measurement indicators of elderly care for nurse practitioners. (c) describing the implication for human capital of elderly care for nurse practitioners and transferring the relevant meanings to the facilities. This study would use the descriptive research method to conduct data collection by the processes of literature review, interviews and surveys. The nurse practitioners were employed by central area teaching hospitals in Taiwan selected as the research samples. The instrument refers to ”assessment of workplace learning competency and climate” to conduct empirical analysis and discussion. The analysis modeling technique is employed to investigate the determinants and outcome of the human capital measurement indicators of elderly care for nurse practitioners in future, which includes non-financial, human resources development, and workplace learning. The results support our expectation that human capital indicators accumulation modes foster the development of quality of elderly care in hospital. Important role of output-based and managerial international human capital are also identified.
