中高齡及高齡慢性阻塞性肺疾病住院病患營養狀態初探

Nutritional Status among Middle-aged and Elderly COPD Patients-A Pilot Study

背景與目的:本研究運用營養不良通用篩檢工具(Malnutrition universal screening tool, MUST)評估非獨居,中高齡及高齡慢性阻塞性肺疾病(Chronic obstructive pulmonary disease, COPD)住院病患營養狀態。了解中高齡及高齡COPD病患營養不良比率及相關因子。方法:本研究採橫斷面設計,以營養不良通用篩檢工具(MUST)評估COPD病患的營養狀態。收集的資料以描述性統計、無母數統計﹝卡方檢定、無母數檢定(曼-惠特尼U檢定)。結果:本研究共93人完成訪談,整體營養不良率為83.1%,中重度營養不良的COPD病患平均年齡高於低度營養不良者(U=203.500, p<0.001)。男性,宗教信仰於營養不良風險分級有顯著差異;女性,年齡於營養不良風險分級有顯著差異。中高齡(45-65歲),性別於營養不良風險分級有顯著差異;高齡(>65歲),年齡、合併肝臟疾病於營養不良風險分級有顯著差異。結論:高齡COPD病患容易出現中重度營養不良現象,年齡、性別及宗教信仰可能影響COPD病患營養狀況,值得後續進一步探討。

Background & Purpose: The study aimed to investigate the nutritional status in middle-aged and aged home-dwelling patients with chronic obstructive pulmonary disease (COPD) by using the Malnutrition Universal Screening Tool (MUST) in order to explore the prevalence and affecting factors of malnutrition among this particular population. Method: The study conducted a cross-sectional study using the MUST to assess the nutritional status in COPD patients. Data obtained in this study were analyzed by descriptive statistics and nonparametric statistics (X^2 test and Mann-Whitney U test). Result: The prevalence of malnutrition among the total of 93 COPD patients reached 83.1%, and the average age of COPD patients with moderate-to-severe malnutrition was older than that of their low-malnutrition counterparts (U=203.500, p<0.001). Significant factors affecting nutritional status were respectively religious belief for male patients and age for female patients. Gender appeared to be a significant factor among middle-aged COPD patients (45-65 years) and age and history of liver disease among aged COPD patients (>65 years). Conclusion: Malnutrition is a common healthy issue among middle-aged and aged COPD patients. Age, gender and religious belief are factors affecting the nutrition status of COPD patients.
