老年原住民個案之照護經驗

Comprehensive Geriatric Assessment - Experience in Taking Care of an Indigenous Elder

摘要


照顧老年族群時,除了身體健康照護外,生活環境與文化背景也是需要考量並融入在長期照護的一環。較少探究的弱勢族群,如全國原住民長者(55歲以上)人數亦從民國103年的88,691人,增加至109年的115,272人,顯示原住民族老人有逐年增加之趨勢。原住民長者的照護應以「在地老化、社區照護」為目標,長照2.0也針對原住民長者做諸多政策之推行,但同時也面臨到許多照護困境。現今資訊發達,外來資訊豐富,臺灣本土的原住民文化逐漸式微,且原住民族都會化現象明顯,原鄉長者的照護人力不足是一大隱憂。本病例報告透過在醫院端照護一位原住民長者所觀察到的醫療照護問題,探討身體疾病、情緒憂鬱、平時生活環境的不適應、及家人的照護壓力和後續照顧等議題,並透過跨領域的醫療團隊幫助個案及家屬建立治療、追蹤及後續照護的方向。

Long-term care for the elderly includes not only physical care and mental wellbeing but also socioeconomic and culture-based considerations. Taking care of underprivileged minority groups, such as indigenous elders, has been a special challenge for the Ministry of Health and Welfare (MOHW) in Taiwan. The number of indigenous elders had escalated from 88,691 in 2014 to 115,272 in 2020, reflecting the severity of aging problems among this population. In spite of the efforts dedicated to this minority group in the MOHW long-term care 2.0 program, language barrier, short supply of caregivers, and lack of public transport service to expedite access to health care remain problems to be overcome. This case report introduced a comprehensive geriatric assessment for an indigenous elder in a hospital-based setting. In addition to the physical health issues, impacts of changes in living environment, burden of family caregivers, and the goal of geriatric care were also assessed in an interdisciplinary way.
