如何運用周全性老年評估改善蜂窩性組織炎住院的老年個案照顧品質

How to Use Comprehensive Geriatric Assessment to Improve the Quality of Care for Elderly Patients Hospitalized with Cellulitis

摘要


高齡長者因病住院時,除了應針對疾病給予適當治療之外,適時透過周全性老年評估了解病患心理社會層面、日常生活功能、營養等各方面是否有異常之處,並安排跨專業的老年醫學團隊予以協助是很重要的。本文介紹一位73歲女性病患因右小腿蜂窩性組織炎,且影響到病患上下樓梯而住院於老年醫學病房。住院之後,除了治療其蜂窩性組織炎,也透過周全性老年評估發現病患其他的問題,如:照顧母親的壓力、糖尿病前期的病史合併體重過重且飲食習慣不當等。透過跨專業的老年醫學團隊成員協助,不僅提供病患相關的長照資源,緩解病患身為照顧者的壓力,同時也對病患進行飲食及營養衛教,避免後續演變成糖尿病而增加日後罹患蜂窩性組織炎的機率,提升病患照顧的品質。

When the elderly are hospitalized, in addition to offering appropriate treatment for the disease, it is important to conduct timely comprehensive geriatric assessment to identify potential abnormalities in patients' psychosocial conditions, daily living functions, nutrition, and other relevant aspects so that the trans-disciplinary geriatric team can provide assistance. This article introduces a 73-year-old female patient who was hospitalized in the geriatric ward due to right lower leg cellulitis affecting her ability to go up and down the stairs. Other than the treatment of cellulitis, the patient received a comprehensive geriatric assessment that helped identify problems like the pressure of taking care of her mother, and a medical history of pre-diabetes combined with overweight and improper eating habits. With the trans-disciplinary intervention of the geriatric team, the patient was able to gain access to long-term care resources for relieving her pressure as a caregiver and receive nutrition and healthy lifestyle education to avoid subsequent development of diabetes and recurrence of cellulitis in the future. Quality of both care and life was improved for the patient.
