目的:本研究的目的有二,其一是了解身心靈賦能團體對減少乳癌婦女焦慮、憂鬱、及提升身心靈健康之成效,再者本研究目標分析乳癌婦女對團体如何幫助他們心理及人際互動問題的主觀看法 方法:本研究設計採類實驗,隨機控制試驗 (randomized controlled trial)。於2005年3月7日至5月16日在台北市某醫學中心乳癌外科門診收案,以隨機方式將對象分為實驗組及控制組,最後完成研究的人數實驗組12人、控制組16人,控制組僅接受醫師門診治療(usual care),實驗組除了接受醫師例行性門診照顧外並加上10週團體治療,每週一次,每次180分鐘;成效的測量包括憂鬱和焦慮的程度以及身心靈健康,於實施團體治療之前後施行測量,以無母數統計分析結果。實驗組個案參與半結構式訪談,了解他們對團體是如何產生療效的看法,質性結果採內容分析法。 結果:根據Mann-Whitney U test檢定發現兩組在減輕焦慮達顯著差異(Z=-2.329,p=.020),結果顯示實驗組個案比控制組個案減輕較多的焦慮,結果建議身心靈賦能團體介入措施能有效的減輕乳癌婦女的焦慮狀態。依據質性的內容分析,身心靈賦能團體產生8層面治療療效因子:指導、人際間學習、情緒宣洩、普同性、團體的凝聚力、利他性、希望、存在主義。這些層面說明療效因子是如何幫助他們在心理及人際互動上的問題。 結論:身心靈賦能團體介入措施能改善乳癌婦女的焦慮狀態。合併身心靈賦能團體與醫師例行性門診照顧能改善乳癌病患於門診的照護品質。
Aims: This study aims have two,one is to understand the effects of body-mind-spirit empowerment group therapy on reducing anxiety and depression and increasing body-mind-spirit well-being among women with breast cancer, the others is study examines their views on what aspects of group therapy worked to help their psychological and interpersonal problems. Methods: This study is a quasi-experimental, randomized controlled trial design. The study subjects were recruited from outpatient department of surgery from 07/03/2005 to 16/05/2005. The subjects were randomly assigned into experimental group and control group. Twelve subjects in experimental group and sixteen subjects in control group completed this study. The subjects in control group received usual care of physician’s treatment at the outpatient department. In addition to usual care, the subjects in experimental group received 10 sessions of weekly group therapy of 180 minutes each. The outcome variables including levels of depression and anxiety, and body-mind-spirit well-being were evaluated at baseline (pre-treatment) and post treatment. Data were analyzed using nonparametric statistics. The subjects in experimental group were invited to participate in a semi-structured interview regarding their perceptions of the change mechanisms in group therapy. The qualitative data were analyzed by using content analysis. Results:The results of Mann-Whitney U test indicated that there was a significant difference in reducing anxiety (Z=-2.329,p=.020)betweenexperimental group and control group. The results suggested that subjects in experimental group appeared to have a better reduction of anxiety than subjects in control group. The result suggested that body-mind-spirit group therapy had effect on relieving anxiety among women with breast cancer. The qualitative analysis yielded eight domains: (1) guide, (2) interpersonal learning, (3) catharsis, (4) universality, (5) group cohesive, (6) altruism, (7) instillation of hope, and (8) existential factor).These domains illustrate how these therapeutic factors worked to help breast cancer women’s psychological and interpersonal problems. Conclusion:Body-mind-spirit empowerment group therapy could reduce anxiety among women with breast cancer. Combination of Body-mind-spirit empowerment group with physician’s treatment could improve the quality of care for outpatient with breast cancer.
