文獻指出茯苓和山藥的萃取物具有抗發炎的作用,並經常用於降血糖的飲食中。本研究旨在評估單一或合併給予茯苓或山藥粗萃取物對糖尿病前期大鼠之腎臟糖化終產物 (advanced glycation end products, AGEs) 的影響。實驗以腹腔注射 20 mg/kg 的 streptozotocin 來誘發大鼠之糖尿病前期,並以管灌方式分別給予茯苓粗萃取物 (0.14 g/kg/day)、山藥粗萃取物 (0.35 g/kg/day),或合併兩者給予連續八週,於實驗期第 0、3、6、8 週時採集血液樣本,而實驗終止時採集肝臟與腎臟進行相關生化分析。其研究結果顯示,單一或低倍劑量合併給予茯苓或山藥粗萃取物相較於控制組,均能顯著降低大鼠在八週時的空腹血糖值 (p˂0.05),並且,所有介入組皆能顯著降低血漿和腎臟中腫瘤壞死因子-α (tumor necrosis factor-α, TNF-α) 和介白素-6 (interlukin-6, IL-6) 的濃度 (p˂0.05)。在腎臟細胞型態學的結果顯示,飲食中給予茯苓或山藥粗萃取物能降低大鼠糖尿病前期腎絲球基底膜增厚的情形。經由免疫組織染色試驗則發現,其能減少腎臟 AGEs 的堆積和糖化終產物接受器 (the receptor for advanced glycation end products, RAGE) 的表現。綜合以上結果發現,茯苓或山藥粗萃取物的介入,能降低體內慢性發炎反應,進而減少腎臟 AGEs 的堆積,可能具有延緩第 2 型糖尿病進展的功用。
Poria cocos and Dioscorea opposite have been considered to present anti-inflammation capability and are used in blood sugar-controlled diets. The proposed study was to evaluate the effect of single or combination of crude extracts from Poria cocos and Dioscorea opposite on renal advanced glycation end products (AGEs) formation in pre-diabetic rats. After induced with 20 mg/kg streptozotocin, the blood sugar levels of rats remained mimic prediabetes status (140-200 mg/dL). Then, single dose of Poria cocos crude extracts (0.14 g/kg/day) and Dioscorea opposite crude extracts (0.35 g/kg/day), combination of both, or vehicle were administered for 8 weeks. Blood and selected organ samples were collected during the study. The results showed 8-week administration of single or combination of these crude extracts could significantly reduce the fasting blood glucose level (p˂0.05) and delay the progress to overt diabetes. The levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) concentrations in plasma and kidney were significantly decreased in administered groups compared to control group (p˂0.05). Such founding are highly correlated with the thickness of glomerular basement membrane. The immunohistochemistry stain showed that the expression of AGEs and the receptor for advanced glycation end products (RAGE) in kidney mainly accumulated in afferent arteriole and the proximal area of renal vessel. These results suggest that administration of Poria cocos or Dioscorea opposite crude extracts could exert an anti-inflammatory effect by reducing AGEs formation and prevent the process of diabetes in pre-diabetic rats.
