紅鳳菜對於糖尿病前期患者血糖控制和抗氧化能力的影響

Effects of Gynura bicolor on Glycemic Control and Antioxidant Ability in Prediabetes

摘要


美國糖尿病學會(american diabetes association, ADA)將血糖值高於標準值但尚未達到診斷標準的族群定義為「糖尿病前期」。若能及早於糖尿病前期時改善其血糖值,可降低罹患糖尿病或併發症的風險。本次實驗的目的為給予糖尿病前期患者每日飲食中含2份紅鳳菜,觀察對於血糖控制和抗氧化能力之影響。共進行12週的試驗,實驗期8週,排空期4週,選入的受試者依照年齡和體位測量數據分為控制組和實驗組,所有受試者於第0 週、第8週及第12週進行體位測量和抽取靜脈血液進行生化檢驗(血糖、血脂、肝腎功能及抗氧化指標等),此外於第0週和第8週給予75克葡萄糖進行口服葡萄糖耐受性試驗(測驗第0、30、60、90、120分鐘的血糖值)、皮膚抗氧化力檢測(彈性和水分散失測試)及繳交三日飲食紀錄(兩天平日,一天假日)。兩組皆給予糖尿病飲食計畫與衛教,依受試者體型和活動量進行個別指導。實驗組每日飲食中含2份紅鳳菜,避免攝取其他紅紫色食物(如:紫米、紅豆、紫高麗、茄子、葡萄、藍莓等);控制組則教育避免攝取所有紅紫色食物。結果顯示,介入8週後實驗組膳食纖維量顯著增加、空腹血糖值降低、改善胰島素阻抗性指標(HOMA-IR)和敏感度指標(QUICKI),提升總抗氧化能力、血中MDA濃度下降及GSH-Px活性上升,推論紅鳳菜因其含有膳食纖維和多酚類使空腹血糖值降低、體內氧化壓力降低進而達到改善胰島素阻抗性和提升胰島素敏感度,可預防發展為第2型糖尿病。

American diabetes association recognized an intermediate group of whose glucose levels do not meet criteria for diabetes, yet are higher than those considered normal as prediabetes mellitus (preDM). Those people indicate has higher risk to develop diabetes in the future. The most common diet recommended for preDM is consuming large amount of vegetable and fruits. Gynura bicolor (GB) is a kind of red-purple colored vegetable, which were commonly found in Taiwan and Far eastern. GB has antioxidant, anti-inflammatory and antihyperglycemic effect in the previous studies. But there are limited studies discussing about the effect of GB on preDM patients. The aim of this study was to assess the effects of serving GB 2 exchanges per day on glycemic control and antioxidant ability on preDM subjects. According to the age and anthropometry data of the participates, we assigned them to control or GB group for intervention period of 8 weeks and washout period of 4 weeks. Data of anthropometry and biochemical analysis were collected at 0, 8 and 12 weeks. Oral glucose tolerance tests, skin test were performed and dietary record at 0 aand 8 weeks. Both groups were received nutrition education and diet plan individually. After intervention, the fasting glucose and malondialdehyde (MDA) values were significantly decreased in GB group. Also, we found that GB group could improve HOMA-IR and QUICKI, increase antioxidant activity and glutathione peroxidase activity. Furthermore, the dietary fiber intake was higher than baseline at week 8 in GB group. There were no difference on blood lipid profile, blood pressure and anthropometric measures. We concluded that GB could improve glycemic control and decrease oxidative stress because of its large amounts of dietary fiber and polyphenols.
