摘要


病歷是民眾在一所醫療機構有關其醫療照護及健康維護之所有資訊、紀錄、報告、影像及文件等以時間軸排序組合而成。病歷的用途即是以專業醫學觀點記錄病患疾病的發生始末,所以醫師必須藉由完整的病歷報告,始能進行診斷過程及確定如何治療,因此病歷可以說是用來描述個人健康狀況的紀錄。 通常電子病歷的呈現較常使用表單來表示資料,患者歷年來的看診情況分佈及用藥情形或手術情形,使用者(醫護人員)比較無法一目了然,也比較不能在第一時間內清楚了解該病患的病情追蹤情況。 以往血管畸形的診斷及治療的多樣性,並沒有一個客觀的標準,若要用臨床數據來做分析研究,不論是哪種研究,時間維度都是必須且重要的,因此我們開發了一套以時間軸為主的電子病歷系統,提供一個創新的方式讓醫師們更方便的瀏覽追蹤患者病情,紀錄每次門診及手術情形,加上術前術後照片,更能精確追蹤治療狀況。使用MongoDB建立資料庫,並用HTML5建置高度互動的視覺化系統。

Medical records is a public medical institution in all information, including records, reports, images and other documents related to the combination with the timeline sort from health care and their health maintenance. Medical reports is use to record the details of a professional medical opinion that occurrence of the disease in patients, doctors can carry out the diagnostic process and determine how to treat, so medical records can be said to describe as a personal health records. Electronic medical records usually presented data in forms, users (medical staff) can not clearly understand patient’s history of visiting doctor’s distribution and drug using or surgical at a glance, and can not clearly understand disease tracking. The diversity in both diagnosis and treatment of extracranial vascular malformation (VM), there’s no objective criteria. It is almost impossible to try to represent and analyze clinical data in the absence of a temporal dimension. So we developed a timeline-based electronic medical record system that provides an innovative way to make it easier for physicians to track the patient's condition, record each patient and surgical situations, and add with preoperative and postoperative photographs, can more accurately track treatment status. Create a database by using MongoDB, and use HTML5 to build highly interactive system.
