腹部症狀選擇X光檢查或內視鏡檢查之評估

摘要


在作腹部症狀的檢查時,可安排X光攝影檢查或內視鏡檢查,如果祇作一項檢查就可達到診斷或治療的目的,在有限的醫療資源下是醫療人員必須努力的方向。 電腦斷層攝影科技的進步,可達到虛擬大腸鏡的影像處理,但終究診斷的功能大於治療的需求,而內視鏡的發展,包括窄頻內視鏡、膠囊內視鏡、經鼻內視鏡以及無痛內視鏡的檢查,增加了診斷的正確性,減少了檢查的不適性,使得一種檢查達到了診斷及治療的目的。 本研究經由署立桃園醫院就診的100位接受X光檢查的病患中就其性別、年齡、教育程度、看診醫師檢查次數、決定方式及決定理由來分析其選擇X光檢查後有無再作內視鏡檢查。 結果顯示,病患之性別、年齡、教育程度、看診醫師是否為腸胃科醫師、決定理由,對於作完X光檢查後是否繼續作內視鏡檢查有顯著差異,至於病人的檢查次數與決定檢查的方式則沒有顯著影響。

In the examination for the abdominal symptoms, we can arrange the X-ray examination or endoscopic examination. Under the limited medical resource, we must try hard to achieve that only one examination can reach the purpose of diagnosis or therapy. The improvement in technology of computed tomography examination, we can present the virtual colonscopic image but the function of diagnosis is larger than the demand of therapeutic. Then, the progress of endoscope, including capsure endoscope, ultrathin endoscope and painless endoscope, can improve the diagnostic accuracy and decrease the discomfort from examination. So, the patient can accept one examination and complete the diagnosis and therapy. In this study, we chose one hundred patients who did examination in Tao-Yuan General Hospital to investigate. From the items of gender, age, education, the frequency of examination, method of decision and reason of decision, we analyze that the patient will do examination of endoscope or not after choosing the X-ray examination. The result showed the most influenced factor is the degree of education. Then, the visited doctor is the specialist. The age and gender had some influence. The examination numbers and decision method had no apparent influence.
