本文以資料包絡分析法(Data Envelopment Analysis;DEA)CCR 及 BCC 模式,衡量某區域醫院各科之技術效率、規模效率與整體效率,透過效率的衡量瞭解醫療資源利用情形及各科間同儕比較找尋最適合各科經營規模與效率,以做為資源分配之參考。投入項為醫師人數、護理人數、醫師薪資,產出項為門診人次、住院人日、醫療收入,決策單位為98年及99年18個臨床科別,98年及99年合併36個臨床科別。 研究結果顥示:98年及99年各有18個決策單位,有7 個科別相對有效率佔38.9%,有11個科別相對無效率佔61.1%。整體效率之平均效率值98年及99年為分別為76.7%及76.2%。 98年及99年二年度有效率科別皆被參考10次的有新陳代謝科及精神科,跨年度精神科(98)被參考24次,新陳代謝科(98)被參考18次,表示精神科(98)經營績效最佳,新陳代謝科(98)次之。 投入項中以刪除醫師人數的效率值差異最大,其次依次為護理人數、醫師薪資;產出項中以刪除醫療收入的效率值差異最大,其次依次為住院人日、門診人次。 關鍵字:資料包絡分析法、區域醫院、經營績效
This article provided Data Envelopment Analysis method ( DEA ) namely the CCRand BCC models, which measure the technical efficiency, scale efficiency and pure technical efficiency of regional hospital through effective measures. Measures were used to understand the utilization of medical resources and peers comparison between subjects and find the most appropriate operational scale and efficiency of various subjects, to be taken as reference for the allocation of resources. From 2009 to 2010 there were a total of 36 clinical divisions, 18 clinical divisions each year which were included in the decision making units that included inputs in terms of the number of physicians, nursing personnel, physician compensation and outputs for the outpatient visits, number of inpatients along with medical income. Research result showed :In 2009 and 2010 there were 18 decision making units in each year, 7 (38.9%) units were relatively efficient, as oppose to 11 (61.1%) units were non-efficient. Within 2 years, the average efficiency value was 76.7% and 76.2% . In 2 years time, the most efficient units were the Endocrinology and Psychiatric division which were reference 10 times. Combining the 2 years, Psychiatric unit(2009) was referred 24 times and the Endocrinology unit(2009) 18 times, making the Psychiatric unit (2009)he best and the Endocrinology unit (2009)coming in second. Regarding input, the biggest difference in efficiency value was in the number of physician followed by nurses, and lastly physician compensation; In terms of output, the largest difference in efficiency value was with medical income, number of inpatients per day, and lastly outpatient visits.
