以免疫組織化學染色法觀察c-Fos在慢性牙周炎患者牙齦組織的表現

摘要


研究陸續發現c-Fos在胚胎發生過程、骨骼分化、神經傳導路徑、長期慢性發炎反應(如糖尿病、類風濕性關節炎、慢性牙周病)與腫瘤形成等領域的相關性。在分子生物學、信息傳遞路徑研究中,大量研究已證實NF-κB與c-Fos對誘導腫瘤生成、慢性牙周病有重要的影響地位,本實驗主要是藉由免疫組織化學染色法,直接觀察慢性牙周病患者的肉芽組織中,c-Fos蛋白質的表現情形,進一步確認c-Fos在慢性牙周病的表現。 材料與方法: 17位慢性牙周病患者(實驗組),與7位牙齦組織健康患者(對照組),利用免疫組織化學染色法,觀察比較c-Fos的表現情形。 結果:在結締組織中,實驗組的平均染色百分比為65.91%,顯著高於對照組的平均染色百分比21.00%,在統計學上達顯著差異(p<0.001)。c-Fos染色強度的情況,在實驗組(17位)結締組織中,以II級(7/17)、III級(7/17)佔82.36%,對照組(7位)結締組織中c-Fos染色強度的情況,以小於或等於I級(6/7)佔85.71%,兩者達顯著差異。 結論:慢性牙周病患者長期承受牙周致病菌慢性刺激,體內的免疫系統與外來的致病菌不斷地在活性期、靜止期的發炎反應信息傳遞路徑中,引發全身性調控因子,包括NF-κB、c-Fos的持續作用。在本實驗中,c-Fos在慢性牙周病患者的表現明顯高於健康者,且達統計學上顯著差異。應用在牙周病的臨床治療時,如何干預發炎信息傳遞路徑是近來研究的重心之一,其中抑制c-Fos活性達到減緩甚至阻止牙周病持續發生,也是未來應用在臨床治療的新思維。

Objective :The purpose of this study was to evaluate the presence of protein c-Fos in the granulation tissue of the periodontal pockets, after non-surgical periodontal therapy. Materials and methods :Test group :Seventeen granulation tissue samples were obtained from periodontal pockets of depth ≧6mm after non-surgical periodontal therapy including removal of plaque and calculus、scaling 、root planing、oral hygiene instruction. Control group :seven healthy gingival tissue samples were obtained from crown lengthening surgery (six)and healthy maxillary tuberosity(one). Both test group and control group samples were removed under local anesthesia during routine periodontal surgery. Immumohistochemical staining with antibody against c-Fos was performed. Results :c-Fos expression increased in the test group when compared to control group. The mean intensity of c-Fos expression in the test group was 65.91% and in control group was 21.00%(p<0.001). Moreover the mean potent of c-Fos expression in test group was increased(p<0.05). Conclusion:These results might reflect a tendency for the high expression of c-Fos in chronic periodontitis even after removal of plaque and calculus、scaling 、root planing、and oral hygiene instruction.
