本研究為描述性研究,目的為探討慢性腎臟病進階護理師角色功能執行現況及困境。以立意取樣,採滾雪球方式取得訪談對象,從2015年2月10日至2015年4月20日,由台灣北部、中部、南部15家參與腎臟病健康促進機構之醫療院所,共20位慢性腎臟病進階護理師,以深度訪談及錄音方式進行資料收集。研究者將訪談錄音轉成逐字稿,並以當前慢性腎臟病進階護理師之角色功能及困境進行資料分析。研究結果發現慢性腎臟病護理師所扮演之角色包括:病患照顧管理者、教育者、諮詢者、溝通協調者、代言者、成果與品質的管理者、慢性腎臟病防治政策活動的執行者及個案發現者等;執行困境有角色定位不清、不合理的人力比及缺乏符合慢性腎臟病進階護師角色能力之教育培訓等困境,使慢性腎臟病進階護理師們,在面臨龐大的個案數及工作量下,時有心有餘而力不足的無奈,難以兼顧照護品質。本研究針對慢性腎臟病進階護理師角色功能及執行困境進行探討,真實呈現其臨床經驗與感受,期能做為慢性腎臟病進階護理師角色拓展及慢性腎臟病防治計劃推行之參考。 關鍵字: 慢性腎病進階護理師、角色功能與困境、描述性研究
Background: Advanced practice registered nurses for chronic kidney disease (CKD-APNs) play an important role in improving the quality of chronic kidney disease patient management. Therefore, it is important to understanding the role and function of CKD-APNs to promote professionalism in this nursing category. Purpose: To explore the role, function, and implementation barriers of CKD-APNs. Method: This descriptive study used in-depth interview semi-structured for data collection, designed by the researcher. Twenty participants of CKD-APNs in Taiwan were interviewed. Result: Eight roles were identified: executive health care manager, educator, consultant, communication coordinator, patient spokesperson, quality monitor, chronic kidney disease prevention policy activity manager, and case discoverer. Three implementation barriers include the following: poorly defined roles, severe lack of personnel and lack of education and training for carrying out CKD-APN role. Conclusion/Implications for practice: This study may be used as a reference for developing training courses and role development for CKD-APNs in Taiwan.
