本研究目的在探討重症護理人員對生命末期照護知識、態度及溝通溝通支持感與溝通自信心相關性探討,以横斷式相關性設計(A cross-sectional correlational design),採方便取樣,以南部地區某醫學中心加護病房為研究場所,選取符合研究條件之對象共229位護理人員。使用結構性問卷進行資料收集,由取樣對象自行填答之問巻進行資料分析。研究資料利用SPSS for wnidow 16.0 中文套裝軟體進行資料分析含描述性統計:平均數、標準差;推論性的統計:獨立樣本T 檢定、Pearson’s 相關係數分析ANOVA等統計方法。 結果顯示年資與生命末期照護知識、態度及溝通時感受到支持的程度及溝通自信心,呈正相關。照護態度及溝通自信心會因是否有重要親屬面臨瀕死經驗的不同有顯著差異。依據Pearson’s相關係數分析溝通自信心與重症生命末期照護知識、態度及溝通支持感的關係,結果發現溝通自信心與知識及態度皆呈現正相關,此外溝通自信心與溝通支持感次量表中的「同事支持」及「環境支持」呈現正相關。此外接受過溝通訓練也會提高護理人員與生命末期病人及家屬溝通自信心,因此緩和療護訓練及溝通技巧訓練仍是有其必要性。
This study aims at probing into the correlation among the end-of-life care knowledge and attitude, perceived level of support during communication and self-confidence in communication of nursing staff in intensive care units. A cross-sectional correlational design and convenience sampling were adopted in this study. A total of 229 nurses from the ICU of a medical center in southern Taiwan meeting the criteria were included in the study. Data were collected using a structured questionnaire, and the subjects were asked to fill out the questionnaire for data analysis. SPSS for window 16.0 Chinese version was used to conduct data analysis, with descriptive statistics including mean and standard deviation and inferential statistics including Independent Samples T-test, Pearson’s correlation coefficient analysis and ANOVA. The results show that seniority is positively correlated with end-of-life care knowledge and attitude, perceived level of support during communication and self-confidence in communication. Significant difference appears in care attitude and self-confidence in communication depending on whether the nurses have a close relative facing near-death experience. Pearson’s correlation coefficient analysis was conducted to analyze the correlation among self-confidence in communication, end-of-life care knowledge and attitude, and perceived level of support. The results show that there exists positive correlation between self-confidence in communication and knowledge of and attitude toward end-of-life care. Moreover, self-confidence in communication is positively correlated with the “Colleagues’ Support” and “Environmental Support” in the subscale of perceived level of support. In addition, communication training would also increase the nurses’ self-confidence in communication with terminal patients and their family members. Therefore, training in palliative care and communication skills is still necessary.
