傳統醫學與另類醫療目前約為全世界80%的人口所應用。隨著人口老化與環境變遷迅速,傳統醫學備受重視,中醫學研究正蓬勃發展。本研究試圖以策略分析及策略制定之概念性模型TOWS矩陣,應用於中醫咳嗽病診治進行探索性研究。 本文以臨床常見之中醫咳嗽為案例進行個案分析,所挑選的三個案例分別為小兒反覆感冒咳嗽、青年感染性肺炎急症以及孕婦久咳不癒胎元不穩,其病情緩急不同,病人稟賦各異。本研究以德菲法進行專家意見之收集與整合,取得咳嗽病之內外部環境定義,以及經過SWOT分析TOWS矩陣策略形成法,有效地取得個案之治法方藥選項,以及依此治法、方藥選項衡量情勢,評估出最適處方。 本文驗證了將TOWS矩陣應用於中醫疾病診治之可行性。本研究結果可以簡化中醫診療的思辨過程,使中醫師可以簡易地獲得正確的醫療方向與思路。本研究亦擴大TOWS矩陣之應用範圍,將企業管理之策略分析工具有效地運用於中醫之診療過程。
Traditional medicine and alternative medicine have been employed by over 80% population of the world. With rapidly increased aged population and environmental changes, much attention has been paid on traditional medicine. Traditional Chinese medicine research is under vigorous development. The present study attempts to explore the application of a strategy analysis and formation concept, the TOWS matrix, to traditional Chinese medicine diagnosis on coughing diseases. Three coughing cases commonly appearing in clinical traditional Chinese medicine were selected. They are a repeated coughing child, a young man infected by acute pneumonia, and a coughing woman with unstable pregnancy. The degrees of the diseases and the patients’ physiological characters are different. The study used the Delphi method to collect and analyze medical doctors’ opinions on internal and external environmental dimensions of selected coughing diseases. Through the SWOT analysis and TOWS matrix strategy formation process, the present study can effectively get appropriate treatment methods and recipes for each coughing cases. Research findings reveal that the TOWS matrix is applicable for traditional Chinese medical diagnosis. The present study demonstrates a simplified diagnosis process for traditional Chinese medicine, and extends the application of the TOWS matrix. Strategy analysis tools can be effectively applied to traditional Chinese medical diagnosis.
