以多變量分析評估音頻共振量測系統之髖關節複合系統鍵結及臨床評估

Assessment and Clinical Evaluation of Total Hip Replacement by Sonic Vibration Technique and Multivariate Analysis

摘要


本研究以非侵入式音頻共振量測技術討論髖關節複合系統之振動訊號及鍵結情形之臨床評估。此外,更進一步證實音頻共振量測系統臨床應用之可能性,並試圖建立人工髖關節複合系統鍵結情形之判斷準則,期望在髖關節鍵結臨床診斷提供較為客觀之判斷準則。 臨床測試以接受過髖關節置換術之病患為受測對象,記錄病患基本資料(身高、體重、腰圍、臀圍、大腿圍等…)及髖關節振動訊號,經由Pearson相關及淨相關 (Partial correlation) 分析進行適用參數的篩選,並以區別分析(discriminant analysis)建立資料判斷式,使鬆脫判斷由定性討論進入定量討論的範疇。 結果顯示,經由淨相關分析,可以擷取出適當的參數,側躺—大轉子組中,鍵結情形—峰值個數偏相關係數為0.5281,呈現正相關;而正躺—前上腸骨脊的淨相關分析中沒有與鍵結情形呈現顯著相關的參數。經由區別分析,側躺—大轉子組中,正確性83.3%、靈敏度89.5%、有效性78.3%;在正躺—前上腸骨脊正確性63.2%、靈敏度64.7%、有效性61.9%。 本研究首次嘗試將多變量分析引入髖關節鬆脫訊號的診斷,並針對先前文獻所提供之HHT及受測姿勢—位置的選擇上加以驗證,結果顯示淨相關分析對於較適當參數的選定有顯著的效力;區別分析則可以直接獲致較適參數與鬆脫之間的關係與權重,使得分析結果得以更加客觀。但是,欲將其應用於臨床醫學,尚需使得系統效能更加優良;此外,系統的縮小化及簡單化亦為努力方向之ㄧ。

This research investigated the clinical evaluations of the complex hip joint system by non-invasive sonic vibration techniques. In addition, it was tried to prove the possibility of clinical applications to set up the criterions of the complex hip joint, and to provide more objective criterions. The subjects were taken from total hip replacement patients. The basic information (ex: height, weight, waist, buttocks, etc...) and the vibration signal were obtained. The suitable parameters were shifted out by Pearson’s correlation analysis and Partial correlation analysis. The connections were set up by Discriminant analysis to make not only the qualitative analysis but the quantitative analysis. In this study the Multivariate Analysis were applied to the diagnosis of the hip loosening. And it was confirmed that the HHT and the measured posture could be applied for this research. This result showed that the more suitable parameters can be got by Partial correlation. Loosening was obtained by Discriminant analysis to relate and weight between parameters and loosening which made the analyzed results more objective. However the efficiency of the system had to be improved much better in the future. Furthermore, the minimization of the system was the important concern before clinical application.
