社區照顧關懷據點使用者付費之研究：200元的背後

A Study of Usercharge in Community Caring-Concern Centers:the Meanings behind 200.

摘要

在社區實務工作上，「使用者付費」概念已有在社區照顧關懷據點中落實，但在文獻上甚少整理社區照顧關懷據點推動使用者付費的經驗，故本研究目的為：一、分別從社區組織、使用者以及公部門的角度去瞭解社區照顧關懷據點推行使用者付費的經驗，以及目前的執行情況。二、透過社區照顧關懷據點推行使用者付費的經驗，整理推行使用者付費對於關懷據點的影響。三、探討比較社區組織、使用者以及公部門對於社區照顧關懷據點中推行使用者付費的觀點與核心價值。
本研究採質性研究方法，透過閱讀文件檔案資料、深度訪談與觀察法來蒐集有效的完整資料。因考慮組織的同質性，故以社區發展協會所成立辦理的社區照顧關懷據點為主，研究對象區分為社區組織、長者以及公部門三方面，採取立意取樣的方式，選取共14位研究對象。
研究發現，社區照顧關懷據點推動使用者付費的原因，主要是因為政府部門對於社區照顧關懷據點的補助不足與項目限制所致，而關懷據點本身的永續經營與自給自足的考量亦是推動使用者付費的重要因素。在社區照顧關懷據點之四項服務中，餐飲服務為主要需要收取費用之服務項目。使用者付費也提供社區照顧關懷據點營運的經濟基礎，提高組織運作自主性。
其中值得注意的是，在社區照顧關懷據點中的費用不是單純的金錢價值而已，而是一種社會價值。社區照顧關懷據點中所收取的費用低於成本與市場價格，是一種補貼性質的象徵性收費，其更重視社區照顧本質與理念的落實，著重人與人之間的連帶關係，分享著彼此的關懷與情感。

In community practical work, the concept of "usercharge" has been implemented in the community caring-concern centers, however, the experience of how to implement seldom raised in literature. The purposes of this study as follows: 1. From the point of views of community organizations, users and the public sector, to understand the way in which the concept of usercharge has been implemented in community caring-concern centers. 2. How far the community caring-concern centers have been influenced by the concept of usercharg. 3. Compare the core values of usercharge in community caring-concern centers, from the perspectives of community organizations, users and the public sector.
The findings of this research included: 1. Owing to the insufficiently and limitation of government subsidy, the concept of usercharge has been used in the community caring-concern centers; 2. Under the consideration of the sustainable and self-sufficiency of the community caring-concern centers, the concept of usercharge has been implemented. Comparing four services in the community caring-concern center services, meal service is the main charge service; 3. usercharge also provide community caring-concern centers operated by the economic base, and improve operational autonomy.
It is worth noting that, the payments charged in community caring-concern centers is not simply the money value, but social value. The payments charged in community caring-concern centers are lower than cost and market prices, with the value of community care concept, focused on the interpersonal relations, sharing a mutual aid and emotions.
