電子病歷交換與其對醫院績效之前因探討： 雲端運算觀點

　　隨著國內醫院電子病歷(Electronic Medical Records, EMR）的實施情況普及，有關電子病歷交換之議題也逐漸受到關注，為了減少醫療資源浪費以及提供更全面性的診斷決策，國內衛生署正積極推動電子病歷交換。國內電子病歷交換尚處於萌發初期階段，相關文獻指出國內醫院對於電子病歷交換意願保守，其原因包括技術面與成本面上的不足，且在台灣健保給付制度的背景下，資訊科技投資對於醫院來說是相對昂貴的。因此本研究期望透過雲端運算的新興IT服務模式，瞭解醫院是否藉由雲端服務的採用，降低電子病歷交換在成本面與技術面上的門檻，進而提升交換意願。研究理論基礎採用科技－組織－環境架構 (Technology-Organization-Environment Framework, TOE Framework)，並加入相關變數來探討影響電子病歷交換意願之前因；此外，欲瞭解電子病歷交換對醫院所帶來之正面效益，提供給經營決策者作為實施參考，本研究亦添加績效構面加以探討。
　　本研究以99年度實施電子病歷交換及互通績優醫院之資訊人員為研究對象，共回收231份問券，有效問卷為83份，有效回收率為36 %，並使用SPSS20.0與SmartPLS2.0統計軟體進行資料分析。研究結果顯示，在科技構面中，雲端服務的採用會正向影響電子病歷交換意願；在組織構面中，高階主管支持會正向影響電子病歷交換意願，正式化程度會負向影響電子病歷交換意願，而醫院規模、集權化程度並無顯著影響；在環境構面中，政府政策與競爭壓力皆對於電子病歷交換意願有正向影響；最後，電子病歷交換則會正向影響醫院績效。

Abstract

With the popular implementation of Electronic Medical Records (EMR), this issue related to its exchange has begun to be concerned and discussed. In Taiwan, The Department of Health is actively promoting EMR exchange, which is still in a germination stage. Some literatures indicate that the intention of EMR exchange is cautious in domestic hospitals, because IT investment is more expensive for Taiwan's hospital in the background of National Health Insurance (NHI). It is the reason of the lack of information technology and cost. Therefore, this study expects to know whether the hospitals adopt Cloud services to reduce the IT and cost obstacle about EMR exchange. The study is based on Technology - Organization - Environment (TOE) framework and the construct of hospital performance to develop an integrated model.
The study chooses the outstanding performance hospitals in 2010 implementing EMR exchange for the sample published by Department of Health, Executive Yuan. A total of 231 questionnaires are mailed to the information technology staffs of medical centers, regional hospitals, and local hospitals. 83 questionnaires are returned and the response rate is 36%. The study use SPSS 20.0 and SmartPLS statistical software for data analysis. The results indicate that Cloud services, formalization, top managers support, government policy, and competitive pressure have significantly effect on the intention of EMR exchange. Finally, EMR exchange has much effect on medical quality and operational performance.
