職業安全衛生管理系統風險評估技術在石油化學工廠應用之研究

摘要


工業安全衛生一直都是產業界所重視的議題,雖然危害鑑別以及風險評估理論及技術不斷推陳出新,但工安意外仍持續發生。每每一件工安災害帶來的不只是硬體及營業上的損失,對罹難者家庭更是帶來無限悲痛,為保護公司財產安全以及作業員工人身安全,一個有效率且易上手的危害鑑別表對降低職業災害是有幫助的。 研究對象C石化廠原已有危害鑑別表,但尚未更新,本研究參考相關安全研究學說理論,勞動部頒布製程安全評估定期實施辦法所認定的五種製程安全評估法,並探討C石化廠近10年高處作業工安意外事故報告,最後依據職業安全衛生管理系統(OHSAS 18001),以及臺灣職業安全衛生管理系統(TOSHMS)對高處作業進行工作安全分析。對各項作業進行危害鑑別與風險評估。 為了使風險評估結果更貼近現場單位,採訪談該事業單位現場資深領班為主,同時也讓第一線人員對危害鑑別表有初步了解,再運用風險評估技術指引,依據訪談所得到的數據資料套入風險矩陣,判斷其風險等級。 依據每一項目風險等級的不同持續監控或擬定改善方法,完成高處作業危害鑑別表。最後鑑別出風險等級最高作業項目為:在高處保養檢修項目進行高處管線盲斷時,因管線內部未確實排空造成有毒氣體外洩。對比實際作業情況,在2017年04月24日即發生與此項作業相同的重大工安意外;隨後事業單位立即檢討改善,聯合高雄市政府舉辦大型聯合演習,向各單位辦理相關訓練及緊急應變演習,並要求各單位將此類作業所造成危害嚴重性,列入操作人員勤前教育的宣導事項內。 本研究危害鑑別表的制定方法亦可運用在硬體方面,建議未來可以持續運用此方法,對於單位各工場設備製程逐一制定,先進行危害鑑別再對單位主管、現場領班及操作人員進行訪談,最後進行風險評估制定該工場專屬的危害鑑別表。

Industry security has been important issues for years. Although the know-how about the danger estimate and risk assessment have been updating all the time. However, relative accidents never stop happening. Every disaster of industry brings not only loss of hardware but also pain to the people who suffer from the disaster. It is important to build an efficient as well as an easy-to-understand system to protect company's property and make sure that staff members are in the safe workplace. This study quotes a raft of widely known theory about safety researching from the literature review and also reviews five different kinds of OSHMS. The latest edition of OSHMS is considered as the main topic in this study to build an efficient form of danger-estimate which is suitable for staff as well as meets laws and regulations and directions of skill for the people who work in the height. This application of research method is comprehensively used even if the small range of researching. It can be applied in the operating of hardware and staff training in Petrochemical Industry. We could also build a flowchart of stakes by this method to analyze the factors of intricate Cross - Strait relations and make the domestic resources used to the extremes as well as face to the weakness to overcome it and make it be dealt with. The structure of OSHMS of this study case has been flawless so far, nevertheless, it's a pity that the updating on the estimate of danger is inefficient. The operation staff doesn't really understand it as well as gets wrongly operated. It causes the discrepancy between the ideal system and the real requirement, it's the main factor to cause disasters. Hopefully, this study can be practicable in the workplace and the accidents can be reduced in the future.
