醫療手術說明同意書之管理系統與數位簽章

摘要


對醫療業而言,因日益增加的資料處理量與複雜度,而引進資訊系統來提昇醫療院所之管理與行政效率,電子病歷即是在此醫療資訊系統發展趨勢下之產物,醫療手術說明同意書也屬病歴的一部分,於是本研究之目的即為建置一醫療手術說明同意書之管理與數位簽章系統,我們首先將手術說明同意書分類建置成為一電子資料庫,醫事人員只需透過系統輸入病患之病歷號碼並選擇所需之手術同意書種類即可新增一份手術說明同意書,並將手術說明同意書之資料內容與輸出樣式分開管理,使用者可依自身需求設計不同的輸出樣式,最後將文件格式轉換成為PDF檔案型態,並應用自然人憑證簽具手術說明同意書,利用數位簽章的特性來確認簽署人之身分以及文件之完整性。

With the development of information technology and its heavy daily life application, the increasing amount of data and its complexity have urged medical industry to introduce information system to raise the efficiency in managing hospitals. Electronic Medical Record is such artifact of the information system for medical industry. The Medical Surgery Consent that a patient signs for receiving the treatment is part of the medical record and should be included in the EMR also. The objective of this thesis is to construct the management and digital signature system for the Medical Surgery Consent. We focus on the Medical Surgery Consent in the EMR to make it electronic and facilitate its digitalization. We also established a database for those Medical Surgery Consents to make and manage the output of the Medical Surgery Consent separately in XML and XSLT formats. Furthermore, we applied Citizen Digital Certificate to realize its digital signature after transforming the documents into PDF files. By using the certificate IC card users can validate the identity of the signer and the completeness of the Medical Surgery Consent, thus assuring the quality of digital signature.
