無效性母乳吸吮行為是普遍存在於嬰兒的餵食問題,母親哺餵母乳時若遭遇到嬰兒吸吮問題而無法獲得改善時,會影響到母親哺乳的自信心,甚至造成母親提早停止哺餵母乳。口腔按摩(oral stimulation)是一種能促進嬰兒口腔吸吮能力的介入性措施,目前臨床上執行對象主為早產兒。本研究目的旨在探討口腔按摩介入對於改善足月嬰兒無效性母乳吸吮之成效,採類實驗研究設計,以中部某區域教學醫院符合『無效性母乳吸吮行為』的足月嬰兒共60名為研究對象,並分派至實驗組與對照組各30名,實驗組嬰兒連續施予3天的口腔按摩措施,對照組則維持一般傳統的嬰兒照護模式,並以早產兒母乳哺餵行為量表(Preterm Infant Breastfeeding Behavior Scale;PIBBS)評估兩組嬰兒在母乳吸吮行為上的差異性。統計分析結果顯示實驗組嬰兒在接受三天的口腔按摩措施介入後,整體母乳吸吮行為包括尋乳反應、能含住乳頭的範圍、含乳持續時間、吸吮、最長吸吮群組以及吞嚥的表現皆優於對照組,並達到顯著的差異性。口腔按摩介入措施能有效改善嬰兒的無效性母乳吸吮行為,本研究結果可提供給臨床專業人員在推展母乳哺餵實務之參考依據,以協助母親提昇哺餵母乳的效能。
Oral feeding difficulty is a common breastfeeding problem in infants. If mothers experienced breastfeeding difficulties such as sucking problems in infants and if these difficulties were not resolved, such problems may discourage mothers from breastfeeding in the future. Oral stimulation is an evidence-based neonatal oral feeding program which serves to promote oral feeding performance in preterm infants. This study was explored the effects of oral stimulation on breastfeeding efficiency in full-term infants with sucking problems. A quasi-experimental design was adopted and sixty full-term infants were divided equally between the experimental group and the control group. A 3-day oral stimulation program was given to the experimental group, while the control group received traditional nursing care. The Preterm Infant Breastfeeding Behavior Scale (PIBBS) was used to evaluate the sucking patterns. The results indicated that oral feeding performance including rooting, areola grasp, latch-on duration, sucking, longest sucking bursts, and swallowing in the experimental group, was significantly higher than that in the control group. This result provided clinicians with valuable information which may help to improve oral feeding performance in newborn infants and promote breastfeeding efficiency in the future.
