為滿足台灣目前長期照顧需求,衛生署於2007年核定「我國長期照顧十年計畫」,推動社區化的長期照護,以在地老化為長期照顧之目標。而2010年台灣地區老年人口已占總人口10.8%,隨著老化所需的長期照顧也以護理為照顧品質的關鍵專業,故本研究旨在探討醫院護理人員對我國長照十年計畫之認知情形;以及了解影響護理人員對長照十年計畫認知之相關因素。本研究以新竹地區某區域教學醫院之護理人員為研究對象,採用橫斷式調查研究法,以立意取樣方式進行問卷調查,共回收有效問卷數300份。研究結果發現: 研究對象以女性居多(98.7%),平均年齡為31.97 6.92歲,婚姻狀況一半以上未婚(52.0%),教育程度以技職大學最多 (54.4%),平均服務年資為7.35 6.39年,專業能力進階層級四成為N2級(40.1%),工作單位以內科病房佔多數(20.7%)。研究對象對我國長照十年計畫之認知情況普遍低落,認知總平均得分為48.54分(滿分100分);護理人員之年齡、服務年資、婚姻狀況、專業能力進階層級、工作單位、目前有無在職進修、家中同住年長者人數、是否為家中年長者之主要照顧者,會影響其認知狀況。本研究建議,加強護理人員在長期照顧方面的教育,個案或家屬是否有長期照顧需求應列為護理人員的交班事項,並依服務年資的多寡與專業能力進階層級的等級,分別規範護理人員每年應接受的在職教育時數與長期照顧課程內涵。
The elderly population has accounted for 10.7% of the total population in Taiwan in 2010. For meeting the long-term care needs in Taiwan, the 10-year Long Term Care Plan was approved by the Department of Health (DOH) in 2007. The project was implemented to promote community-based long-term care and to faciliate the goals of aging in place. Nursing is the key professional service associated with the quality of long-term care in the country. This study aimed to assess the awareness of long-term care policies in Taiwan and to understand the factors affecting the awareness among nursing staff. The nurses of a regional teaching hospital in Hsinchu area were included in this study that conducted cross-sectional surveys with purposive sampling. A total of 300 valid questionnaires were collected and found to be dominated by females (98.7%). The participants had a mean age of 31.97±6.92 years and most were unmarried (52.0%). Those with an educational attainment level of technical and vocational colleges were predominant (54.4%). The average length of work experience was 7.35±6.39 years. Most participants had professional competence level of N2 (40.1%) and those worked in medical wards accounted for 20.7%. However, the participants’ awareness of long-term care policies in Taiwan was generally low as indicated by the total average score of 48.54 out of 100 points. The factors associated with nurses’ awareness consisted of age, years of work experience, marital status, professional competence level, work unit, on-the-job training, the number of older people living together at home, and being the primary caregiver of the elderly at home. On-the-job training could enhance the awareness of nursing staff. These findings suggest that survey for patient or their family with long-term care needs should be listed in the nursing items during each shift change. It is also necessary to provide nursing staff with on-the-job long-term care training courses for a given legnth of time defined according to years of work experience and professional competence levels.
