南投縣基層公共衛生人員之職場疲勞評估研究

中文摘要 背景與目的:基層公共衛生人員在台灣衛生工作推廣與落實過程中扮演相當重要的角色,在平日對病患及社區民眾宣導健康議題的同時,是否忽略了自身的疲勞問題及健康狀況是一個值得我們關心的議題。本研究旨在評估基層衛生所員工之職場疲勞現況及其影響因素。 研究方法:本研究採普查方式,以南投縣13 個衛生所員工為研究對象,以面對面方式進行自填問卷調查,共發出195 份問卷,回收有效問卷179 份,回收率91.8%。採國內研究者鄭雅文(2008)、李明濱編製且信、效度均佳的量表為主要研究工具,含:中文版職場疲勞量表、工作心理負荷量表、工作控制量表、就業不安定性量表、心理困擾量表。 研究結果:護理人員的職場疲勞較其他服務職別嚴重;平地鄉衛生所人員的職場疲勞明顯高於山地鄉;高度工作心理負荷及低度工作控制是影響衛生所員工職場疲勞的重要因素;在「整體職場疲勞」的迴歸模型中,達到顯著的疲勞預測因子為工作滿意度、心理困擾程度、工作心理負荷、衛生所地區型態、服務職別、工作控制、自覺健康,解釋力58.65%。 結論與建議:考慮地區型態的差異,定期檢討調整單位內人力配制及工作內容,提昇工作滿意度。並針對高危險族群作宣導與介入,改善員工心理困擾、減輕工作心理負荷感、促進自覺健康。

Abstract Background and Purpose: Basic-level public health workers have played critical roles in the health promotion in Taiwan and the implementation process. When advocating health issues to patients and community members, the ignorance of individual burnout and health conditions is worth concerning. This study aims to evaluate the present situation of workplace burnout of the workers in basic-level health centers and the factors. Research method: With general survey, the staff in 13 health centers in Nantou County was selected as the research subjects. By face-to-face questionnaire survey, total 195 questionnaires were distributed and 179 valid ones were retrieved, with the retrieval rate 91.8%. The applied research tools, created by Cheng (2008) and Lee and presenting good reliability and validity, contained Workplace Burnout Scale, Psychological Work Demands Scale, Job Control Scale, Job Insecurity Scale, and Psychological Disturbance Scale in Chinese version. Research outcomes: The research findings showed that nursing staffs presented more serious burnout than the ones in other service positions. Workers in health centers in plane villages perceived higher burnout than the ones in mountain indigenous townships. High mental workload and low job control were the major factors in workplace burnout of the staff in health centers. In regard to the regression model of occupational burnout, the predictive factors of work satisfaction, psychological disturbance, local style of health center, mental workload, service position, job control, and health awareness achieved remarkable burnout, with the explanation 58.65%. Conclusions and Suggestions: In consideration of local styles, regular examination and adjustment of the human resource distribution and the work contents in the unit could be preceded from the aspect of promoting work satisfaction. Furthermore, propaganda and intervention in high-risk groups could improve psychological disturbance of staffs, reduce metal workload, and enhance health awareness.
