摘要

本研究目的為探討影響台灣醫院實施電子病歷之因素，以台灣中部四縣市的醫院為對象，研究方式係採結構式問卷與深度訪談法，擬定『探討影響台灣醫院病歷電子化之因素調查問卷』，總寄發112 份調查問卷至各醫院，回收有效問卷54份，回收率達48%，並訪談10 位負責電子病歷之單位主管，藉此瞭解各醫院對電子病歷概念的認知，及各醫院病歷電子化的情形與發展電子病歷系統時遭遇的問題，期能對國內發展電子病歷的現況做描述。研究發現：由於現階段處於政府推行初期，不易羅致有相關經驗之資訊人員，各醫院都面臨到專業人員培訓不易的問題。各醫院尚未推行電子病歷最主要影響因素為建置經費不足，政府單位對於各醫院所面臨之問題應大力協助克服，提供相關經費補助，以加快各醫院全面推行電子病歷。醫院實施電子病歷除了可減少儲存空間，便利病歷之調閱與應用，減少尋找病歷的時間外，對於整體醫療體系有助於跨院之轉診及建立病人資訊交換的基礎，全面提升醫療照護品質之外部效果，因此，政府對於各醫院所面臨之問題應大力協助克服並提高誘因。

The purpose of this study was to explore the factors affecting electronic medical records in hospitals in Taiwan. The questionnaire were mailed to information departments of 112 hospitals ,total number of respondents were 54 response rate is 48%, and face-to-face in-depth interview was given 10 electronic medical records directors .To understand the concept of electronic medical records, and development problem of electronic medical records system. The findings of this study were: now the government carry out a policy in the early stage, hospitals are faced with difficult to recruit IT staff and offer train of professionals. The most important factors for not yet implementation of electronic medical records is fund, the government help to overcome the problem and provide fund to implement electronic medical records. The convenience of access to medical records and applications, reduce time of looking for records, and establish the basis information exchange, raise the overall quality of medical care .Therefore, the government should help to overcome and increase financing incentives for all hospitals.
