長期照護工作人員對機構住民隱私維護的探討

摘要


國內高齡人口快速增加,照護人力的需求量更是有增無減,其中護理人員、照顧服務員仍屬長期照顧體系中最大照顧人力,但因勞力密集,工時長、機構工作人員流動率高,其專業素養對機構住民隱私維護的照顧品質值得堪慮。故本研究目的是要探討長期照護工作人員對機構住民隱私維護之重視度;以及不同屬性之長期照護工作人員對機構住民隱私維護的差異。本研究屬橫斷式探索性研究,採立意取樣,以中部地區縣市立案之長期照護機構共14家之長期照護工作人員為研究對象,運用商機演算法,於套裝軟體PASW18.0進行描述性統計、獨立樣本t檢定、單因子變異數分析及複迴歸分析。研究結果發現影響長期照護工作人員對機構隱私維護的重要因素中,以身體隱私維護最為重視、也最困難執行,顯示問題聚焦後更可以激發品質改善的動力;另外也發現愈正向的勞動條件,工作人員對住民隱私維護的重視度也愈高,如薪資、工作時數、服務年資等。由此可見商機演算法可以找出工作人員認為最需要協助的部分;建議機構管理者可以運用績效獎勵,與工作人員在此部分發揮巧思運用機構有限資源,促進對住民隱私維護的品質。

As Taiwan’s population of elderly residents grows rapidly, the demand for related medical services has been on the rise. Among these, professional nursing and caretaking personnel make up the majority of the long-term caregiver force. However, the intensity of demand, long working hours and high staff turnover rates have posed a serious threat to both the quality of professional caretaking in organizations and to residents’ privacy. The objective of this study is to explore how long-term caretakers view the privacy of their charges. In addition, it will probe how the nature of different kinds of long-term caretaking affects the way staff maintain this privacy. This research has adopted a cross-sectional study judging sampling method and targeted long-term caretakers at 14 organizations in central Taiwan. Opportunity Algorithm Analysis and PASW18.0 were utilized to conduct descriptive statistics, independent samples t test, one way analysis of variance and multiple regression analysis. Study results showed that although long-term caretakers placed the highest value on maintaining physical privacy, this was the hardest aspect to practice, making it one of the crucial factors in regard to organizations’ maintenance of privacy. This in turn demonstrates that such data can become a force toward quality improvements. This study also found that the value caretaking staff placed on their charges’ privacy had a lot to do with wages, working hours and the benefits provided by their organizations. It’s clear that Opportunity Algorithm Analysis can help locate areas of assistance that staff members are most in need of in their work. It also suggests that management staff in these organizations can implement a reward system to encourage caretakers to raise privacy standards, utilizing existing resources within the organizations.
