下顎間葉性軟骨肉瘤－病例報告
Mesenchymal Chondrosarcoma of the Mandible-A Case Report
摘要
間葉性軟骨肉瘤(mesenchymal chondrosarcoma)是一種在軟組織及硬組織都很罕見的惡性腫瘤。組織學上呈現贅生的軟骨細胞散佈於間葉細胞中的雙相性(biphasic picture)。手術切除是目前治療的首要選譯，放射及化學治療則是扮演輔助的角色。此惡性腫瘤具有高度侵犯性及遠處轉移的能力，患者的預後不佳。本病例報告中的患者為一41歲女性，最初因為右下顎牙齒動搖，對冷飲敏感及該區牙齦刷牙時易流血，及右下顎無痛性腫脹已持續數月等問題而至本院就診。經切片檢查診斷為下顎間葉性軟骨肉瘤。術前檢查並無遠處轉移之情形。故患者於2003年6月接受手術，術後並定期回院複診。回顧英文醫學文獻中僅有約93個發生於顎骨的間葉性軟骨肉瘤被發表，故特此提出報告。

Mesenchymal chondrosarcoma is a rare malignancy occurring both in hard and soft tissues. One of the predilection sites of the tumor is the facial skeleton, especially the jaw bones. Histologically, it has a characteristic biphasic picture of islands of neoplastic cartilage amid highly vascular spindle-celled or round-celled ”mesenchymal” tissue. It is aggressive with high potential to metastasize and poor prognosis. We herein present a 41 y/o female patient whose initial thief complaints were painless swelling over right mandible, mobility, bleeding tendency and hypersensitivity to cold fluid of teeth over that region. Incisional biopsy revealed mesenchymal chondrosarcoma of right mandible. Further pre-operative examinations did not show distant metastasis. She underwent surgical intervention in June, 2003 and has come back for follow-ups periodically without recurrence or metastasis thereafter. Only 93 cases occurring in the jaw bones have been reported in the English literature. The rarity of this malignancy warrants further investigation.
