摘要


為能探討國內醫院醫療品質指標使用情形,本研究以全民健保特約醫院(扣除精神專科醫院)575家為對象,在民國87年3月至5月採郵寄問卷追訪兩次,並佐以電話核對資料方式進行調查,共回收有效問卷195份(回收率33.9%)。指標之選取主要參考國內相關文獻較常用的21品質指標,結果發現最常被醫院採用的品質指標為佔床率(88.2%)、平均住院日(84.1%)、粗死亡率(71.8%)、醫師病床比(69.2%)、院內感染率(67.2)、醫護人員病床比(63.6%)及剖腹產率(50.8%)。醫院評鑑層級愈高,愈多醫院會使用品質指標來管理醫院。在不同評鑑層級醫院所使用之品質指標值,大部分呈現出顯著差異。建議衛生署應依不同層級醫院在醫療照護所扮演的角色,建立一套合乎國情之醫療品質指標申報及管理系統,以監督醫院醫療照護品質以確保病患照護。地區教學及地區醫院有很高比率的醫師門診人次超過60次,可能會造成醫師無充分時間與病患溝通及灌輸病患正確之醫療觀念知識與行為,進而影響醫療品質及病患之求醫習慣。

To assess the quality of hospital health care in Taiwan, we used mailing twice survey to the hospitals contracted with National Health Insurance. The questionnaire covered 21 indicators. A total of 180 hospitals complete the survey (a response rate of 33.9%). Occupancy rate(88.2%), average length of stay(84.1%), crude death rate(71.8%), physicians per bed(69.2%), nosocomial infection rate(67.2%), physicians and nurses per bed(63.6%) and cesarean section rate(50.8%) were the indicators popularly be used in hospitals. Medical centers used more indicators than other rating level of accredited hospitals. There were different in many indicator value among different accreditation hospitals. District teaching hospital, and private district hospital still have a very high proportion(>50%) of physicians have more than 60 patients in each OPD clinic. This study suggest that Department of Health should set the quality assurance indicators to monitor the quality of health care and patients' outcome in hospitals.
