本研究以回溯方式,統計『全民健康保險醫療院所轉診單』中『原診治醫院診所』之各項資料填寫情形,其提供填寫及修訂內容之參攷。自89.12.01至92.11.30間,經由轉診至本院急診室之患者總計13870位,其中,11013(79.4%)位患者於轉診程序中,原診治之醫院附有衛生署制定之『全民健康保險醫療院所轉診單』。筆者回顧接獲之轉診單發現:醫療院所對於轉診單中『原診治醫療診所』之各要項(包括:保險對象資料、病歷摘要、轉診日期、有效日期、建議轉診院所名稱等統計21欄)資料填寫情形不一。統計結果顯示:各欄位中,以病歷摘要中這『診斷』一欄之填寫率爲最高(97.8%);病歴摘要中之『病情摘要』一欄之填寫率(97.1%)雖略低於前述幾項,然於統計學上並顯著差異。填寫率顯著偏低之項目中,以『建議轉診院所地址』之填寫率爲最低(3.65%)。此外,轉診單之『有效日期』及原診治院所之『傳真號碼』之填寫率亦僅達9.45%及25.5%。本研究僅統計轉診單中『原診治醫療診所』之各項資料填寫情形,然對其未能詳盡填寫之原因則未探討,期後繼之研究能擴大範圍並分析原因,以使轉診單之內容更切合臨床需要。
The medical referral system is a prerequisite to ensure the quality of medical care as well as control of the health care expenditures. According to the policy, document related to the patient is a necessity when patient is referred. Therefore, the referral sheet plays an important role while referring. Two major components are included in the referral sheet, the first part, which includes 21 items, should be filled out by the referral physician, and the other, by the consultant. The purpose of this study is to evaluate the completion of the first part of referral sheet of National Health Insurance. A retrospective study has been done in our hospital, which is a 1300-bed medical center. Data was collected since December 2000 to November 2003. The overall patients referred from other hospitals to ours were 13870 and 11013 (79.4%) of them brought a referral sheet. According to the results, referral diagnosis and patient's general information such as name, sex as well as date of birth are filled out more frequently. The completion of address of consultant hospital, valid date and fax number are far behind others. We suggested that it's important to educate physicians to properly filled it out and is probably necessary to re-evaluate the components of the referral sheet.
