跨專業牙科治療：保留自然牙齒，還是選擇植牙？

人工植牙在二十世紀蓬勃發展以來，對牙科治療與患者的生活品質產生了莫大的影響。牙周專業治療也不再侷限於保留自然牙齒，而是積極投入植牙手術與相關之軟硬組織重建。然而當臨床上面臨保留自然牙齒，還是選擇人工植牙的兩難時，首先應考慮患者的條件與需要。其次應評估各式牙周與植牙治療的預後與長期穩定性，然後再為患者擬定理想的治療計畫與其他替代計畫。過到複雜病例時，除牙周專業之外，常常需要贗復、齒顎矯正、根管治療、口腔外科等其他專科的協助才能得到最理想的治療效果。本報告將以臨床實際病例，仔細說明遭遇嚴重牙周病患者時，如何與其他專業共同合作，依現有各類牙周治療方式保留牙齒，或選擇人工植牙。

Dental implant had a great progress in the twentieth century and also had a tremendous impact on dental therapy & patient’s life quality. The goal of periodontal therapy is not only to preserve natural dentition but also to restore missing teeth by implant surgery and associated hard & soft tissue reconstruction. When facing the dilemma of preservation of natural tooth or extraction of hopeless teeth and replacement with dental implants, patient’s surgical condition and needs shall be the most important factor to consider. In addition, the predictability and long-term stability of various periodontal & implant therapy should be evaluated before preparation of the ideal or alternative treatment plans for the patients. When more complicated case were encountered, in addition to periodontal therapy, other specialties may be needed to achieve most ideal treatment outcome. The purpose of this paper is to compare periodontal & implant therapy and use clinical case reports to explain how multidisciplinary approach was applied for the treatment of periodontally-compromised patient with either periodontal or implant therapy.
