植牙與自然齒列：從牙周再生治療觀點

摘要

二十世紀末牙周再生治療因其可修複過去認為已經喪失卻無法抗回的牙周破壞組織而大行其道，甚至有些牙周嚴重破壞的牙齒接受牙周再生治療後免於被拔除的命運。這種治療模式在不久後受到人工植牙漸趨成熟而受阻，因為人工植牙泛用宣稱可取代狀況不良的牙根。本篇文章的目的嘗試從一位臨床醫師的角度使用交叉比對牙齒預後（prognosis）及戰咯價值（strategic value）的方式，對於嚴重牙周病的牙齒提供選擇給予牙周再生治療或以人工植牙取代的判斷原則，並探討各種牙周再生治療的複原其臨床上質與量的義意和限制。牙周病醫師在為患者謀取最大福利和嚴守保存自然齒列最高天職的同時應當審慎評估。

Periodontal regeneration has gain much attention in late twentieth century in its ability to restore what was once considered lost for good-the injured periodontium. In some instances, it serves to rescue certain severely endangered natural teeth due to periodontal upset. This treatment modality has gained overwhelming acceptance until it met strong resistance from the emergence of dental implant which claims to replace natural tooth if they re not betteroff.The purpose of this paper is to provide a different perspective from a clinical practitioner point of view utilizing cross matching determination of “prognosis and strategic value” to justify the use of periodontal regeneration and/or dental implant for tooth with critically compromised periodontium. The clinical significance and limitation of periodontal healing by different regeneration procedures were also discussed. Holding these two diverse treatment modalities at hand, we as periodontists should exercise at our best discernment to satisfy the ultimate interest of the patient as well as to fulfill the given responsibility as a clinician, that is, to preserve the natural dentition as much as we can.
