To save natural teeth or to place implants, it is always a difficult decision for the most clinicians. The concepts of saving natural teeth whenever possible have been a fundamental principle of dentistry. However, many studies have confirmed an excellent long-term prognosis for implant treatment. Attempts to save the questionable teeth may not be as necessary as in the past, and should be reconsidered and evaluated for the optimal treatment plan. This paper tries to discuss the risk factors of replacing the compromised teeth with osseointegrated implants from the Prosthetics view. It is emphasized that esthetics and function of the final implant-supported restorations must be considered and planned in the beginning. If risk factors of the implant can not be resolved, preservation of the tooth would be considered the treatment of choice.
