護理之家照護品質之先驅性研究

A Pilot Study for the Quality of Care in Nursing Home

摘要


本研究目的在探討護理之家,住民、家屬、護理人員與病患服務員所認為的護理之家照護品質,並分析四者感知間的異同點。採質性研究法,以中部地區某醫院附屬護理之家為研究場所,隨機抽取住民、家屬、護理人員與病患服務員各三人,利用半結構式問卷進行深度訪談。研究發現四者所認定的護理之家照護品質可分成五大類組:結構層面、直接照護、基本人權、人際互動與照護成果。每一類組又可細分為數個次類組,每一次類組皆包含一些照護品質單元。以照護品質單元出現的頻率來分析住民、家屬、護理人員與病患服務員對護理之家照護品質看法的異同。結果發現:住民、家屬、護理人員及病患服務員四者對護理之家照護品質的感知,以結構層面此類組最多,基本人權類組最少。以FISHER’S EXACT統計分析不同受訪者所感知的護理之家照護品質類組,結果發現無顯著差異,即四者的看法無不一致情形。研究結果除了可以增添現有護理之家照護品質的實証外,亦可供日後擬訂護理之家照護品質評估量表的參酌。

This study was undertaken to explore and compare the perceptions of care quality in nursing home from the perspectives of different groups including residents, families, nurses and nursing aids. Twelve participants were recruited from one nursing home in Taichung City, three participants for each group. A qualitative research method and in-depth interviews were used to collect data. There were five categories to describe the care quality in nursing home: structural aspect, direct care, basic human right, interpersonal interaction and outcome of care. Each category was divided into several subcategories, and each subcategory included many care quality units. With regard to the similarity and difference of nursing home quality, the subjects reinforced the care quality in structural aspect and least mentioned the basic human right. Using FISHER’S EXACT statistic method to analyze the nursing home quality perceptions, there were no difference among the four groups. The results of this study may help the long-term care professionals to understand the perceptions of care quality for nursing home in residents and families. Nursing interventions are, therefore, adjusted to relate more appropriately to residents’ and families’ perspectives. Additionally, the results can provide common indicators for establishing the instrument of care quality in nursing home.
