台灣是個地小人稠的島國,自1971年以來,癌症已經躍居國人首位死因,且癌症死亡人數正年年增加中。安寧運動起源於1983年,第一個安寧機構設立於1990年,目前已有29個通過評鑑的安寧病房,及40個居家服務單位。三個基金會、一個協會和一個醫學會已經加入推廣安寧運動。目前的安寧服務收容量僅能滿足部份需求。教育訓練仍是最迫切的需要。而學術研究最重要的是要建立本土化的模式。近日,安寧緩和醫療已計劃正式列入醫學院校教育課程中。政府有關單位已大力支持並規劃安寧療護,及提供多項資源。安寧療護住院及居家的評鑑工作由台灣安寧緩和醫學學會開始執行,同時,2001年第一批的安寧緩和醫療專科醫師誕生了。在台灣安寧運動的現況是相當積極的也是極有潛力的。安寧照顧已擴允涵蓋到非癌症的末期病人。本文係以2003年的觀點回顧及展望台灣安寧緩和醫療狀況。
Taiwan is an island country, with a small area and a dense population. Cancer has been the leading cause of death for over 20 years, and the number of cancer deaths is increasing years by year. The hospice movement started in 1983, and the first hospice was set up in 1990. There are twenty-nine qualified in-patient units and fort home care services at present. Three foundations, one association and one academy have joined in with the promotion of hospice movement, The current hospice-service capacity only meets a few needs of the demand site. Education together with training is still the top necessity. Research is highlighted with an indigenous model. The curriculum of palliative medicine has recently been confirmed for education in the future. The government resources has been of great help in the development of hospice palliative care, and more service needs are identified. Accreditation of in-patient and home care units was initiated from 2000. first group of Specialist Palliative Medicine was also qualified in 2001. The status of the hospice movement in Taiwan is positive and promising. A plan of extension service to cover the non-malignant terminal illness is emerging.
