台灣呼吸治療師工作情境與執業能力之德懷先驅研究

A Delphi Study of Working Situation and Practicing Competency of Respiratory Therapist in Taiwan

摘要


本研究採用修正型德懷術(modified Delphi method),探討台灣地區區域層級以上醫院,臨床呼吸治療師工作情境以及所須的執業能力。本研究以文獻回顧、資深治療師訪談、以及職場實地觀察,初步收集和歸納工作情境與執業能力的資料,編製爲德懷第一回合問卷。本研究選取20位呼吸治療師專家,共經二次問卷往返達成共識。兩回合問卷以眾數、平均數、標準差以及四分位差進行意見的分析,並以語意七分量表(semantic scale)請專家們評量各項目的重要性與常見性。本研究結果共歸納9類70項的工作情境與9類123項的執業能力。工作情境方面,70個項目(100%)的工作情境皆被認爲高度重要,其中有44個項目(62.8%)是屬於高度常見的。執業能力方面,有118項(96%)被認爲是高度重要的能力,其中有81項(66%)是被認爲高度常見的。因此可以了解,呼吸治療師的工作不僅是非常的重要,而且工作的發生頻率也相當高。於三階段之能力分類中,雇用時能力有6類29項,起始期能力有9類61項,後續發展期能力有6類15項,尚有7類18項無法歸類。據此三階段之能力,可做爲學校教育或在職教育規劃課程時的依據。

The study is to explore the working situations and practicing competencies of respiratory therapists in the hospital in Taiwan. The entire research was proceeding with two rounds by performing modified Delphi method. Round 1 questionnaire was developed via collecting data from literature review, senior respiratory therapists interviewing, clinical observation recording for respiratory therapists. Twenty experts from clinical respiratory therapists, team leaders, chiefs, and school teachers were invited. All experts reach agreement through two rounds. The collected data of two rounds were analyzed by modes, means, standard deviation, and interquartile range. The importance and occurrence of every item was scoring with a 7-point semantic scale by experts. The results that were totally nine categories including seventy items of working situation and nine categories including one hundred and twenty-three items of practicing competencies. Advance in analysis of competency, 6 categories and 29 items were categorized in hiring stage, 9 categories and 61 items were categorized in initiating stage, 6 categories and 15 items were categorized in ongoing stage, and 7 categories and 18 items were not in any stage. Seventy items of working situation (100%) were valued highly in importance, and forty-four items (62.8%) were valued highly in occurrence. One hundred and eighteen items of practicing competencies (96%) were valued highly in importance and eighty-one (66%) items were valued highly in occurrence. Therefore, we realized that respiratory therapist's jobs were not only highly important but also highly occurring. We could plan programs of school education or on-job training according to our study outcomes.
