Experience of Treatment of Varicose Vein of Legs Using Sclerotherapy
使用硬化劑與彈性繃帶壓迫包紮於治療表淺性靜脈性曲張之經驗

自1991年1月至1997年7月，共有253例表淺性靜脈曲張患者，在台大醫院及國泰醫院接受硬化劑治療，注射後以彈性繃帶壓迫包紮兩星期，再穿彈性襪兩個月。這些患者以前都未曾接受過任何手術或硬化治療。 患者在接受硬化劑治療前，都接受Perthe's Test的檢查，以便排除患者有深部靜脈栓塞的可能性。所有患者都在門診接受治療。所使用的硬化劑是3% Sodium tetradecyl sulfate(STS; Sotradecol)，每個表淺性靜脈曲張處注射的Sotradecol量都在1~ 2毫升。硬化劑注射後，接受治療的下肢就以彈性繃帶包紮起來，以便壓迫注射過的靜脈。 接受治療的患者，年齡從33歲至77歲，平均是52.3歲。每側下肢接受注射硬化劑的總量從2毫升至12毫升，平均是8毫升。治療的門診追蹤與電話訪談，顯示治療效果很好，治療過的表淺性靜脈曲張都會消失，只有少數患者發生沒有注射部位的復發。至於併發症，有119位患者在注射部位有短暫性色素沉著（佔47%），有83位患者在注射部位有短暫性硬塊（佔33%），這些併發症大都在治療後6個月內自然消退。依我們的經驗，使用硬化劑壓迫包紮治療表淺性靜脈曲張，是個簡單易行且效果很好的方法。

From Jan, 1992 to Jun. 1997, 253 patients of superficial varicose veins were treated with sclerotherapy, in National Taiwan University Hospital. The patient who had undergone any previous surgery or sclerotherapy were excluded. Before sclerotherapy, the Perthes' test was done to exclude the possibility of deep vein thrombosis. Without anesthesia, most of patients were treated at the outpatient clinic. Sodium tetradecyl sulfate (STS; Sotradecol) 3% was used as the sclerosant. Usually the average of 1cc of the sclerosant was injected to each marked site of varicose vein. The compression dressing was immediately applied over the lower extremity following injection of the sclerosant. The age of the patients ranged from 33 to 77 years with the mean age of 52.3 years. The amount of sodium tetradecyl sulfate injected to each lower extremity ranged from 2 to 12 ml, and the average amount was 8 ml to each lower extremity. The results of sclerotherapy were delightful. The majorities of varicose vein have subsided, except some residual lesions need further one or two treatment in few cases. The complications included temporary hyperpigmentation of injected site of skin in 119 cases (47%) and temporary induration of injected veins in 83 cases (33%). Remarkable nodular thrombolic lesion were noted in 20 cases (8%) which are considered to be uncooperative group who did not follow the instruction strictly after the sclerotherapy. Most of the hyperpigmentation and induration subsided gradually and spontaneously within 6 months after sclerotherapy.
