Using Second Toe Plantar Flap in the Reconstruction of Finger Tissue Defect

摘要

從1998年五月至八月，桃園敏盛綜合醫院整形外科利用第二趾腹面自由皮瓣重建五位手指皮膚缺損之病患，結果令人感到滿意，自從Kimata在今年一月美國重建整形外科醫學雜誌發表這種方法後，我們認為第二趾腹面自由皮瓣後之外觀及功能都可以達到比其他手術更好的結果。其好處包括
1．皮瓣薄，方便重建手指特別外形。
2．顔色，質感與正常手指相近。
3．可以用時伴隨神經以重建觸覺。
4．其動、靜脈大小與手指血管相當，容易吻合。
5．取皮瓣區不會留下明顯的疤痕。
在本文中會進一步探討第二趾腹面自由皮瓣重建手指皮膚缺損之特點與手術方法。

Five cases of finger tissue defect reconstructed with second toe planter flap are presented. The advantages of this procedure are that it provides both aesthetic and functional result at the recipient site and little morbidity of the donor site. The flap receives its vascular supply from the second toe lateral or medial plantar digital artery with a short pedicle, or the first metatarsal artery with a long pedicle if the condition of the digital artery of the injured finger is not satisfactory for micro-surgical anastomosis. Venous flow is obtained from the subcutaneous plantar or dorsal vein. Nerve is obtained from the lateral or medial plantar digital nerve. All 5 cases reconstructed with this procedure revealed satisfactory result.
