Using amoxicillin, we evaluated the efficacy of an antimicrobial agent in the clearance of H. pylon and the healing of duodenal ulcer (DU,), and we investigated the pathogenic role of H. pylon in DU. Forty-nine DU patients with positive urease test were randomized to receive 4 weeks of treatment with amoxicillin or with placebo, following a double-blind study design. At the end of the therapy, 75.0% (21/28) of the amoxicillin-treated patients showed H. pylon clearance. This was significantly higher than in the placebo group (9.5%, 2/21) (p<0.001). Healing of duodenal ulcer occurred in 75.0% (21/28) of the amoxicillin-treated patients, whereas only 19.0% (4/21) of the placebo-treated patients showed healing (p<0.001). Among the amoxicillin-treated patients, 15 of the 21 patients with healing ulcer obtained clearance of H. pylon, yet 6 of the 7 patients who failed to clear H. pylon also showed ulcer healing after four weeks of treatment (p>0.4). Grades of antral gastritis improved significantly after 4 weeks of treatment with amoxicillin (2.15±0.66 vs. 1.15±0.46, p<0.01). These results suggest that H. pylon is possibly an important etiological factor in duodenal ulcer.
