Electronic Root Canal Length Measurement-A Literature Review

摘要


成功的牙髓病治療,其先決條件必需要有一個正確的根管操作長度,才能對根管進行有效擴大修形及適當充塡。臨床上能夠提供根尖狹窄處及根尖孔位置的測量方法,目前僅有電子根管長度測定法擁有此項功能。電子根管長度測定器發明至今近40年,有所謂傳統第一代:由單一測定周波數測定阻抗值的測定器,如Endodontic Meter、Sono-Explorer系列;其次號稱第二代:雙(多)測定周波數的Apit (Endex)、Root ZX等,利用阻抗値的差値或商値比設計,可克服第一代不適用於高導電環境的缺點;近年來更將第二代測定器的Root ZX與機械帶動或超音波帶動的器械結合成具有快速運轉,自動測試長度及自動停止功能的第三代多功能組合儀器:Tri Auto ZX、Solfy ZX、Denta Port ZX等。本文將針對電子管長度測定器的發展做一個回顧性的深入探討。

An accurate measurement of the root canal length allows for an efficient root canal preparation and root canal filling, and eventually determines the success of endodontic treatment. So far, only an electronic method is available for precise detection of apical constrictions and the apical foramen. Electronic root canal measuring devices for canal length determination were introduced over 40 years ago. Three generations of electronic devices have been developed. The first-generation (traditional) electronic devices evaluated the length of the root canal by detecting the impedance induced by a single frequency, e.g., Endodontic Meter, Sono-Explorer, etc. However, the striking disadvantage of traditional devices is that the meter underestimates the length of the root canal length if there are strong electrolytes in the root canal. Overcoming some shortcomings of traditional devices, second-generation electronic devices, including Apit (Endex), Root ZX, etc., simultaneously measure the impedance of 2 different frequencies, and calculate the deviation or quotient of the impedances. Recently, combining the second-generation device Root ZX with mechanical or ultrasonic unit, third-generation electronic devices were introduced. This upgraded design provides several advantages such as rapidity and autodetection, and the file can be stopped at any desired position on the devices display. The third-generation devices include Tri Auto ZX, Solfy ZX, Denta Port ZX, and so on. The practical value of the electric method is well recognized by experts worldwide. However, the principle and classifications of electronic devices remain diverse. Herein we focused on the principles, accuracy, advantages, the history of development, influencing factors, clinical instructions, related studies, and the expected future of electronic devices, and thoroughly review these topics for electronic devices of root canal length measurements in this article.
