Management of Chylous Leakage Following Neck Dissection-Cases Report

Chylous leakage is a rare complication following radical neck dissection, but it may cause delayed wound healing, nutritional impairment, and extended hospitalization. Patients often become nervous when they note large amounts of milky-like drainage from an operative site. Several modalities to treat chylous leakage have been reported, such as ligature of the leaking duct and application of fibrin glue to the leaking region, and ”oversewing” of the possible leakage site using a muscle flap. The use of chelating agents (tetracycline) intramuscularly and medium-chain triglycerides (MCTs) via total parenteral nutrition (TPN) has also been reported. However, effective treatment modalities still remain controversial. In this report, we present 4 chylous leakage cases and categorize the different treatment modalities. An algorithm for the management of chylous leakage is provided.
