This was a case that a traumatic #21 of a nine-year-old child was diagnosed extrusive luxation and crown fracture. After 3 months, pulp necrosis was observed and apexification was performed with the paste of calcium hydroxide and sodium hypochlorite. The radiopaque barrier was visible after four-month treatment. Obturation was performed by lateral condensation. No sign and symptom was observed during six-month follow-up. Conclusion: Monitoring the change of pulp vitality is key factor in the treatment of traumatic teeth and the apexification with calcium hydroxide has been proven. A long term success is depended on the regular follow-up.
