部份體積假影對於牙科電腦斷層掃描影像判讀的干擾病例報告

摘要

由於電腦斷層掃描對於照射物體是以影像重組的方式呈現，所以當被照射物體由不同物質所組成時，因為不同物質X光吸收係數不同，所以當身體器官內相鄰組織X光吸收係數差異很大時，電腦斷層掃描的影像與真實組織大小就會不同，稱為部份體積假影。此效應會讓臨床牙醫師對於牙科電腦斷層掃描影像產生誤判。本報告的第一病例是病患的左上側門齒(22)根尖囊腫進行根尖手術，術前的牙科電腦掃描(Dental CT)影像中所評估的22根尖病變區域與上顎竇之間的齒槽骨體積，小於手術時所見之真實體積。第二病例是病患的右上第一小臼齒(14)側位牙周囊腫，在術前牙科電腦斷層掃描影像中發現囊腫中間有明顯的放射阻射物體，但在手術移除時卻沒有見到相似的物體。本報告顯示在根尖手術前對於牙科電腦斷層掃描影像的判讀，必須注意不同X光吸收係數的組織，所產生的部份體積假影的干擾。

The image of dental computer tomography reconstructed each sliced piece of image. Because the different radiographic absorption of the different oral structures, the ”partial volume artifact effect” interferes the dental CT image reconstruction. The report of two cases describes the false findings of the cyst of the upper lateral incisor (22) and lateral periodontal cyst of the upper first premolar (14). We should understand the possibility of false message from the dental CT image, and correct the errors in the computer reconstructed program.
