利用強力萌出術合併牙冠增長手術改善前牙齒質不足－5年追蹤病例報告
Combining Forced Eruption and Crown Lengthening Surgery to Improve Insufficient Anterior Tooth Structure- A Case Report of 5-year Follow-up
摘要
蛀牙或牙齒斷裂在臨床上可藉由矯正強逼萌出術（forced eruption）、牙冠增長手術（crown lengthening surgery）或合併兩者的方式來增加臨床牙冠長度，以達成理想假牙製作的要求。前牙區在美觀的考量下，矯正強逼萌出術合併牙冠增長手術是較適合的治療方式，以避免所做的假牙牙冠長度過長，或是手術後導致的黑三角（black triangle）以及牙齒牙齦邊緣高度與鄰牙不對稱。 先利用矯正強逼萌出術將斷裂線或蛀牙邊緣提升到上皮附著上方，再使用牙冠增長手術稍作修整牙齦與齒槽骨，讓牙齒最後能夠獲得適合的假牙修磨邊緣（finishing line），以達成理想假牙製作的要求。本篇病例報告乃合併矯正強逼萌出術與牙冠增長手術，並藉由此種治療方式保留了前牙牙根，也恢復了患者美觀與功能。

Dental caries or tooth fracture can be clinically treated by forced eruption, crown lengthening surgery or the combination of both ways to increase the length of the clinical crown to achieve ideal prosthesis. Considering the anterior esthetics, forced eruption combined with crown lengthening surgery is a more appropriate treatment to avoid the too long prosthetic crown, or black triangle and asymmetric gingival margin. First, extrusion of such teeth by using forced eruption allows elevating the fracture line or caries margin above the epithelial attachment, and then the crown lengthening surgery is performed to slightly trim the gingiva and alveolar bone. The teeth can finally get the appropriate finishing line to achieve the requirements of ideal prosthesis. The case report retained the anterior teeth by a combination of forced eruption and crown lengthening surgery, and restored the patient's esthetics and function.
