犬原發性非生殖器型傳染性花柳性腫瘤之臨床研究

A Clinical Study of Primary Extragenital Transmissible Venereal Tumors in Dogs

摘要


傳染性花柳性腫瘤(TVT)是一種主要藉由交配傳播,常發生於犬外生殖器的腫瘤,包括母犬陰道及公犬陰莖與包皮。除此之外,該腫瘤不但會發生於頭部、頸部、四肢或軀幹之皮膚或皮下組織,也可見於口腔、鼻腔和眼結膜的黏膜上。但如果同時無生殖器官病灶者,可視為原發性非生殖器型TVT。報告中所述的7個病例均係取自國立中興大學獸醫教學醫院之門診病例,包括6隻雄性、1隻雌性,患犬年齡介於2至10歲。腫瘤組織為單獨發生或多發於不同部位,存在時間至少2週至6個月。病發組織之存在部位包括皮膚3例(其中1例腹腔內也有腫瘤)、鼻腔3例(其中1例口腔也有腫瘤)和眼結膜1例,經臨床細胞學或/及組織切片檢查診斷為TVT。臨床症狀,除可見明顯團塊外,於鼻腔者,會出現間歇性噴嚏、鼻排出物、流鼻血、開口呼吸、睡眠時鼻鼾和前肢皮毛被血染污等。腫瘤存在於口腔者,可見黏液血樣排出物。各病例均以外科手術切除腫瘤組織或/及以化學療法vincristine治療。所有病例在治療後均至少追蹤1年,皆未出現復發情形。因此利用臨床細胞學和組織切片檢查診斷犬原發性非生殖器型TVT和以外科手術或/及化學療法治療,是有效的處理模式。

The transmissible venereal tumor (TVT) of the dog is mainly transmitted by mating that usually affects the external genitalia, including the vagina in the female, and the penis and prepuce in the male. Besides the skin and subcutaneous tissue of the head, neck, extremities or the trunk, it also occurs on the mucosa of oral cavity, nasal cavity and the conjunctiva. The primary extragenital TVT (PEGTVT) is the major suspect if there is no evidence of primary genital tumors. This paper describes 7 cases of PEGTVT that were presented at the Veterinary Teaching Hospital, National Chung Hsing University. The cases included one female and 6 male dogs with ages ranged from 2 to 10 years. The TVTs were presented as either a solitary or multifocal lesions at different sites with a history of 2 weeks to 6 months. The tumors were observed in the skin (3, 1 also in the abdominal cavity), nasal cavity (3, 1 also in the oral cavity) and the conjunctiva of the eye (1). These tumors were also diagnosed as TVTs by histopathological examinations. Clinically, in addition to the prominent TVT masses, dogs with lesions in the nasal cavity showed intermittent sneezing, nasal discharge, epistaxis, open-mouth breathing, noisy respiratory sounds during sleep and blood-tinged haircoat of the forelimbs. There were mucosanguineous discharges from the oral cavity lesions. All the cases were treated with surgical excision or/and chemotherapy with vincristine. No recurrence of the lesions was detected during more than a one-year follow-up period. Therefore, the PEGTVT can be diagnosed with cytology and histopathology, and effectively treated with surgical excision or/and chemotherapy.
