上顎第二臼齒C型態牙根－病例報告及文獻回顧

摘要

C型態牙根多數發生於下顎第二大臼齒，然而發生於其他部位則非常少見。本文所報告的病例，為具C型態牙根的上顎第二大臼齒，因嚴重牙周病而拔除，經由包埋、橫切後，觀察根管系統由根尖到齒頸的變化。這顆牙齒牙根的外型為C型的型態，頰側可見一線性深凹，根管在根尖處是分開的。根尖孔為3個獨立開口，顎側閉口偏向近心側。在根尖l/3處根管系統，為3個獨立之根管。漸向咬合面，可見顎側根管分成2支，而後顎側2支根管再合而為一形成片狀型態之根管。至於近心根管及遠心根管則是向根尖直到齒頸部獨立的根管。X光片上可見三個根管之投影，顎側根管偏短，與一般臼齒之X光片影像不同。並討論C型態牙根X光片上的影像判讀，C型態牙根根管系統的分類，及臨床上此類牙齒牙周、牙髓治療的考量，以提供給臨床醫師參考。

C-shaped root canals are mostly found in mandibular second molars and are rare in maxillary molars. This report presents a maxillary second molar with a C-shaped root. The periapical radiograph showed 3 individual canals, with the palatal canal being the shortest. The tooth was extracted because of advanced periodontitis. The morphology of the root canal system was studied by embedding the tooth in a resin block and marking cross-sections. The root of the tooth had a C-shaped morphology with a deep grove bucally. There were 3 individual canals and foramens. The palatal canal was divided into 2 parts in the middle 1/3 of the root, and become one in the cervical 1/3 portion. The prevalence and morphology of C-shaped root canals in the maxillary were also reviewed and are discussed.
