Positional plagiocephaly has been ascribed to a preferential sleeping position in early infancy. Quantitative analysis can provide clear information for estimating the degree of deformity by computing the relative data on the head shape. We developed an ellipse shape model to score the asymmetry of the head shape. Then a plagiocephaly index and a profile of concave/convex index were used to describe the symmetry of the head shape. The plagiocephaly index is a useful tool for evaluating the symmetry of the head shape. The profile of concave/convex index is also a useful profile to express local protrusions of the head shape. The natural development of positional plagiocephaly should be understood before undertaking any treatment.
