搖頭眼振對空勤體檢之意義

The Clinical Significance of Head-shaking Nystagmus Test in Aviation Medical Examination

搖頭後出現的前庭性誘發眼振,由於此法誘發率高,故近年來常用於篩檢測驗。航醫中心從1993年1月至1994年12月,兩年間對889名飛航人員作搖頭眼振檢查,其中本國籍584人,外國籍305人,本國籍中505人是空軍飛行員或陪訓飛行員,79人是飛航管制員,外國籍則全爲現職駕駛員。結果有60名出現搖頭眼振(6.6%),其中本國籍飛行員22名(3.7%),外籍27名(8.8%),飛航管制員11名(13.9%)。60例出現搖頭眼振中,有44倒是單相眼振,16例是雙相。單相眼振中31例出現向右眼振,13例出現向左。如果未施行搖頭眼振檢查,則只有12例在閉眼時出現眼振,高達48例出現偽陰性。外籍飛行員出現較高的搖頭眼振陽性,考慮是加齡因素引起,和椎基底動脈循環不良有關。飛航管制人員因未有嚴格的體格篩選,雖然平均年齡只有25.4歲,亦出現高達13.9%之陽性率。

In recent years, the head-shaking nystagmus (HSN) test has been used widely for the detection of vestibular asymmetry because of its high elictation of nystagmus. From January 1993 to December 1994, 889 subjects (505 domestic pilots, 305 foreign pilots and 79 air traffic controllers) were received HSN test at CAMC and there were 60 subjects (6.6%) showed HSN, among them 22 subjects were domestic pilots (3.7%), 27 subjects were foreign pilots (8.8%) and 11 subjects were air traffic controllers (13.9%). In these 60 positive cases, 44 cases were monophasic nystagmus, and 16 were bi-phasic. There are only 12 cases show nystagmns in the condition of closing eyes if HSN test is deleted, that is 48 cases reveal false negative. In foreign pilots group which showing higher provocation rates than those of domestic pilots are considered as aging factor (49.1:37.2), because vertebro-basilar insufficency is the main cause of vertigo in the elderly. Air traffic controllers are just selected by literal test not by strict physical examination, there are 13.9% HSN rates with average of 25.4 ages.
