Simultaneous bilateral spontaneous pneumothoraces (SBSP) is a rare clinical disease, and prompt diagnosis can be difficult as symptoms range from a mild decrease in breath sounds to severe shortness of breath with respiratory failure. We report a 20-year-old slim male presented to the emergency department with sudden onset of severe dyspnoea. Patient was successfully intubated and ventilated with 100% oxygen. Followed up chest radiograph revealed bilateral tension pneumothoraces. Finally, he was discharged from the hospital and his course of recovery was uneventful.
