Spontaneous pneumoperitoneum is an uncommon disease in the emergency department (ED) that defined as the non-traumatic presence of intraperitoneal free gas without an associated demonstrable lesion or evidence of peritonitis. Conservative approaches may be considered if patients with minimal clinical signs, particularly when peritoneal signs, fever and leucocytosis are absent. This article presents the diagnosis and emergency managements for a 38-year-old male patient with spontaneous pneumoperitoneum after his scuba diving.
